The assessment of Buerger’s exercise on dorsal foot skin circulation in patients with vasculopathic diabetic foot ulcer by using wireless near-infrared spectroscope: a cohort prospective study

Abstract: The improvement of peripheral circulation is crucial for the care of vasculopathic diabetic foot ulcer (DFU). Near-infrared spectroscopy (NIRS) was used to investigate the effects of Buerger's exercise on dorsal foot skin circulation with novel technique of measuring the absorption difference in wavelength of light between oxy-hemoglobin (HbO) and deoxy-hemoglobin (HbR). Patients with vasculopathic DFU who could tolerate Buerger's exercise three times a day at home for at least 8 weeks were enrolled (group A).… Show more

“…The DCU is one of the most common and serious complications of diabetes, with a quarter of patients developing a foot or skin ulcer. Each year, 50–70% of patients requiring amputations due to non-traumatic injuries are patients with DM ( Lin et al, 2018 ). The prevalence of diabetic ulcers in Asia, Europe, and Africa is 5.5, 5.1, and 7.2%, respectively ( Boulton et al, 2005 ).…”

“…The wireless near-infrared spectroscope (NIRS) was reported to be used for assessing Buerger's exercise on dorsal foot skin circulation in patients with vasculopathic diabetic foot ulcers [19], cerebral ischemia, and cerebral perfusion pressure in an animal model of traumatic brain injury [20], and a cardiac arrest model for muscle oxygenation, mental stress levels, etc. [21].…”

“…The light emitting-detecting probe was designed to be placed in contact with the morbid limb of the dorsal foot, provide a dual-wavelength near-infrared light source, and detect diffusely reflected near-infrared light. 7,8 A near-infrared light-emitting diode (SMT735/850, EPITEX, Japan) and photodiode (PD15-22 C/TR8, EVERLIGHT, Taiwan) were used for providing the dual-wavelength near-infrared light source (<3 mW) and detecting the reflected light, respectively. The wireless signal acquisition module was designed to amplify and digitize the reflected light and then transmit the signal to the back-end laptop through Bluetooth.…”

“…Recently, we also applied NIRS in estimating foot vascular status and predicting wound prognosis of diabetic foot ulcers. 7,8 In the present study, we used TRANCE-MRI to determine whether venous thrombi are present in patients with static ulcer and analyzed tissue perfusion by using a wireless wearable NIRS device. 7,9…”
