The Application of Head-Up Tilt Test to Diagnose Hemodynamic Type of Orthostatic Intolerance in Children Aged Between 3 and 5 Years

Abstract: Objectives: The head-up tilt test (HUTT) is a useful tool to assess autonomic function and to reproduce neurally mediated reflex. In this study, we evaluated the use of HUTT in pediatric patients aged 3–5 years with orthostatic intolerance.Materials and Methods: The medical history and HUTT records of 345 (180 males, aged from 3 to 5 years) cases of patients who complained of symptoms of orthostatic intolerance and who visited the Syncope Ward, Children's Medical Center, The Second Xiangya Hospital, Central So… Show more

“…Positive responses of the patients were due to VVS, postural tachycardia syndrome (POTS), and orthostatic hypotension (OH). Vasovagal syncope was defined as syncope or near-syncope, simultaneously accompanied by the following hemodynamic changes: a BP decrease [systolic BP (SBP) ≤ 80 mmHg and/or a diastolic BP (DBP) ≤ 50 mmHg or a mean arterial pressure decrease ≥25%] and/or bradycardia (HR <50 bpm) or cardiac arrest >3 s ( 9 11 ). Vasovagal syncope was further classified into three responses based on the classification of the Vasovagal Syncope International Study (VASIS) ( 12 ): mixed type (VASIS I), cardioinhibitory type (VASIS II), and vasodepressor type (VASIS III).…”

“…Head-up tilt test (HUTT) is used for assessing patients with suspected syncope but lack of confirmed diagnosis after an initial assessment, or a differential diagnosis of convulsive syncope from epilepsy or pseudo-syncope from VVS [17]. The head-up tilt period is performed under two conditions: passive (without any provocative drugs), and pharmacological (with drug challenge).…”
