The application of a numerical scoring system for evaluating the histological outcome in patients with chronic hepatitis B followed in long term

Abstract: A numerical scoring system was applied and compared with conventional histological classification to assess the histological outcome in 42 patients with chronic hepatitis B followed for 16 to 162 months (mean = 75 months). Four histological categories in the biopsies were assessed and scored: (i) piecemeal necrosis; (ii) lobular necrosis; (iii) portal inflammation, and (iv) fibrosis and cirrhosis. “…The histological scoring system designed by Knodell et al provides a means of studying the prognostic significance of different histopathological parameters in serial liver biopsies (3). This scoring has been found practical in a long-term follow-up study of chronic hepatitis B where the PMN and fibrosis scores correlated well with the routine histological classification and where a high PMN score was predictive of a later progressive development of fibrosis (4).
