The antisynthetase syndrome

Abstract: Antisynthetase syndrome is an autoimmune disease affecting multiple organ systems that was first described as a triad of polymyositis, interstitial lung disease, and the presence of autoantibodies to aminoacyl-transfer ribonucleic acid synthetases. We describe a patient who developed respiratory symptoms following fume exposures. Following a prednisone taper for presumed hypersensitivity pneumonitis, he developed extrapulmonary symptoms suspicious for an autoimmune process discovered to be antisynthetase-assoc… Show more

“…In clinical trials, mycophenolate mofetil was the best option for immunosuppression, especially in patients with inflammatory NSIP in idiopathic inflammatory myopathies. In our patient, 9 months after the diagnosis and initiation of treatment with prednisone 30mg/day and mycophenolate mofetil 1500mg/day, a significant clinical improvement was obtained, with slight improvement in respiratory function tests and maintenance of NSIP changes on HRCT 5,6 .…”

“…Retesting was prompted due to relapse of symptoms during corticosteroid tapering or discontinuation. Recently, Robinson et al [38] described a case of a 22-year-old man, with progressive dyspnea, working for a company producing ink for printers with several fume exposures, and an initial diagnosis of HP. Due to the development of arthritis during corticosteroid tapering, serologic testing was ordered that was positive for anti-Jo-1.…”
