The Anticoagulant Effect of Heparinoid Org 10172 During Haemodialysis: An Objective Assessment

Abstract: SummaryThe heparinoid of natural origin Org 10172 has anti-factor Xa activity but minimal anti-thrombin activity, and little effect upon broad spectrum assays such as the KCCT in vitro. Its anticoagulant effects have been compared to those of commercial heparin in 7 patients undergoing haemodialysis for chronic renal failure. Commercial heparin was administered in a dose (5,000 iu bolus + 1,500 iu/hour continuous iv infusion) previously shown to inhibit fibrin formation during haemodialysis. This produced mean… Show more

“…All subjects had a normal coagulation screen (see details of tests below) and platelet aggregation studies were within the pattern we expect for the majority of healthy subjects of this age and sex. Platelet counts (in whole blood and in platelet rich plasma) were within the normal (Ireland et al, 1986;ten Cate et al, 1984ten Cate et al, , 1985Turpie et al, 1987) (also see Mikhailidis et al, 1984Mikhailidis et al, , 1985b for additional literature).…”

“…In view of the results reported by other authors using LMWH in hemodialysis, the advantage of Fraxiparin is its use as a single intravenous bolus injection at the start of the session whereas an additional continuous in fusion is usually recommended with UFH [3,16,[20][21][22][23]. This is due to a greater half-life of Fraxiparin as compared to standard heparin.…”

“…Full suppression of fibrinopeptide A generation requires anti-Xa levels of 0.7-1.0 U/nil [28]. At a regimen with anti-Xa levels in the range of 0.4-0.6 U/ml using a heparinoid.…”
