The Anterior Sphenoid Wall: Relative Anatomy for Sphenoidotomy

Abstract: A total of 53 anatomical specimens of the posterior ethmoid and the adjacent anterior sphenoid sinus wall were examined in reference to the operative guidelines for endonasal sphenoidotomy. Six anatomical points of measurement were defined for clinical orientation, and both the absolute and the relative widths of each third (i.e. of each of three vertical sections) of the pars nasalis and the pars ethmoidalis of the anterior sphenoid sinus wall determined. The choana proved to be the most valuable regional an… Show more

“…During the surgery, the drill and optical fiber would be inserted into the nasal passage where the optical fiber would illuminate the sphenoid bone (located at the rear of the nasal septum), which has a thickness of approximately 2 mm. 15 Intraoperative photoacoustic images will be acquired to visualize hidden blood vessels as surgeons are drilling to remove the sphenoidal bone.…”

“…An optical fiber, coupled to a laser, would be attached to a surgical tool. During surgery, the tool and fiber will be inserted into the nasal passage where the optical fiber would illuminate the sphenoid bone which has a thickness of 0.4-8.8 mm [24,25] . Intraoperative photoacoustic images will be acquired to visualize blood vessels and sphenoidal bone.…”

“…In 98–100% the natural ostium is found medial to the base of the superior turbinate [ 333 ], [ 334 ], [ 335 ], [ 336 ], [ 337 ], [ 338 ], [ 339 ]. The distance to the choanae in caudal direction amounts to 21 ± 6 mm [ 338 ] or 2–15 mm [ 334 ]. The distance to the nasal septum in medial direction is only few mm, to the inferior edge of the posterior part of the superior turbinate is mostly less than 10 mm [ 334 ], [ 338 ].…”
