The anterior dislocation of the sacroiliac joint: a report of four cases and review of the literature and treatment algorism

Abstract: Pelvic fractures are an uncommon injury in pediatric trauma patients, but the morbidity and mortality associated with these injuries can be profound. Of the posterior pelvic ring disruptions, the posterior dislocation of sacroiliac joint, which is the traditional dislocation of the sacroiliac joint, occurs in most incidences of pediatric trauma patients. There are few reports, however, on the "anterior" dislocation of sacroiliac joint, in which the ilium dislocates anterior to the sacrum and often combines wit… Show more

“…The following discussion is based on the best way to achieve an anatomical reduction of the dislocation with a solid fixation. Zhang et al [2] have successfully treated four cases of anterior SI joint dislocation using an ilioinguinal incision to obtain a reduction and used a fixation with iliosacral screws, in association with an external fixation for anterior lesions.…”

“…In a rare situation, the fracture or dislocation can be opposite with an anterior slipping of the ilium. Only a few cases have been reported in the children's population [2] and no strong therapeutic consensus is available at the moment.…”

“…It is, therefore, essential to stabilize and restore the oval-shaped osseo-ligamentous structure of the pelvic ring. Unstable fractures of the pelvic ring are an increasingly frequent outcome of motor vehicle trauma [1, 6]. Sacroiliac disruptions are common among unstable fractures of pelvis.…”

“…Displaced and unstable pelvic fractures are commonly associated with disruption of the osteoarticular junction of the sacroiliac joint [1]. Posterior sacroiliac dislocation occurs in most of the instances but there are few reports only for the anterior type of sacroiliac dislocation, wherein the iliac bone fractures and displaces anterior to the sacrum often in combination with fractures of the pubic rami and concomitant symphyseal injuries [26]. …”

“…T o the best of our knowledge, only four adult cases of pure anterior sacroiliac dislocation have been reported worldwide, with an additional six pediatric cases reported in recent years [1][2][3][4][5][6][7] . The few reported cases all describe patients with substantial concomitant injuries, resulting in major permanent disability.…”
