Intravenous regional anesthesia (IVRA) using lidocaine provides effective localized analgesia but its duration is limited. The mechanism by which dexmedetomidine enhances lidocaine IVRA is unclear but may involve modulation of hyperpolarizationactivated cyclic nucleotide-gated (HCN) channels. Materials and Methods: Lidocaine IVRA with varying dexmedetomidine concentrations was performed in the tails of Sprague-Dawley rats. Tail-flick and tail-clamping tests assessed IVRA analgesia and anesthesia efficacy and duration. Contributions of α 2 adrenergic receptors and HCN channels were evaluated by incorporating an α adrenergic receptor antagonist, the HCN channel inhibitor ZD7288, and the HCN channel agonist forskolin. Furthermore, whole-cell patch clamp electrophysiology quantified the effects of dexmedetomidine on HCN channels mediating hyperpolarization-activated cation current (I h ) in isolated dorsal root ganglion neurons. Results: Dexmedetomidine dose-dependently extended lidocaine IVRA duration and analgesia, unaffected by α 2 receptor blockade. The HCN channel inhibitor ZD7288 also prolonged lidocaine IVRA effects, while the HCN channel activator forskolin shortened effects. In dorsal root ganglion neurons, dexmedetomidine concentration-dependently inhibited I h amplitude and shifted the voltagedependence of HCN channel activation. Conclusion: Dexmedetomidine prolongs lidocaine IVRA duration by directly inhibiting HCN channel activity, independent of α 2 adrenergic receptor activation. This HCN channel inhibition represents a novel mechanism underlying the anesthetic and analgesic adjuvant effects of dexmedetomidine in IVRA.

Rejyonel intravenöz anestezi (RİVA), ekstremite operasyonlarında yaygın şekilde yıllardır kullanılan, basit ve güvenilir bir yöntemdir. Çalışmamızda, RİVA tekniği ile kullanılan ilaçların operasyona başlangıç süresi, peroperatif analjezi ve postoperatif analjezi süreleri açısından karşılaştırmasını yapmayı amaçladık. Gereç ve Yöntemler: Çalışmamız üst ekstremite operasyonu öncesinde RİVA tekniği uygulanan hastaları içeren retrospektif kohort çalışmasıdır. Grup T'ye, %2 lidokain 3 mg/kg'a ek olarak tenoksikam 20 mg; Grup D'ye ise %2 lidokain 3 mg/kg'a ek olarak deksketoprofen 50 mg uygulanmıştır. Kayıtlardan preoperatif, peroperatif ve postoperatif dönemde kalp atım hızı, ortalama arter basıncı, oksijen saturasyonu, görsel analog skala [visual analogue scale (VAS)], motor blok başlangıç süresi ve ilk analjezik gereksinim zamanları karşılaştırıldı. Bulgular: Gruplar arasında preoperatif, peroperatif ve postoperatif değerlendirmeler sonucunda kalp atım hızları, oksijen saturasyonları ve ortalama arter basıncı değerleri arasında anlamlı fark yoktu. Gruplar arasında motor blok başlama süresi ve ilk analjezik ihtiyaç süresi arasında istatistiksel olarak anlamlı fark gözlenmedi. Grup D ve Grup T arasında VAS değerlerinde preoperatif ve peroperatif 30. dk'larda istatistiksel olarak anlamlı bir fark gözlenmese de postoperatif 60. dk'da Grup T'nin Grup D'ye göre daha etkili olduğu saptandı. Sonuç: RİVA'da adjuvan olarak kullanılan tenoksikamın, deksketoprofene göre postoperatif dönemde daha etkin bir analjezi sağladığı görüldü. Sonuç olarak, RİVA'da lidokaine tenoksikam 20 mg ve deksketoprofen 50 mg ilave edilmesi arasında preoperatif ve peroperatif analjezi kalitesinde anlamlı bir fark yoktur, ancak postoperatif dönemde tenoksikam daha etkili bir analjezi sağlar.

Aim: Intravenous regional anesthesia (IVRA) is not commonly preferred in the lower extremity because of the toxic risks of high-dose local anesthetics. This study aimed to compare the use of two different concentrations of anesthetics with additional tourniquet application to reduce local anesthetics amount during the IVRA method for short-term foot and ankle surgeries. Material and Methods: In this prospective study, 40 patients were allocated to two groups with different concentration formulations of 200 mg lidocaine hydrochloride (Group 30 and Group 20). The groups were compared in terms of demographic data, tourniquet pain, operation time, hemodynamic indicators, and sedo-analgesia needs. Results: Demographic data were similar in the two groups. The mean tourniquet pain time was 41.66±6.61 minutes in Group 20 (n=9) and 36.76±7.17 minutes in Group 30 (n=13) (p=0.120). Perioperative sedo-analgesia consumptions were similar between the groups: weight-adjusted before/after tourniquet pain (p=0.390, p=0.207, p=0.536, and p=0.176), weight-adjusted/none total amount (p=0.425, p=0.578, p=0.268, and p=0.612), per minute before/after tourniquet pain (p=0.075, p=0.506, p=0.354, and p=0.055), for propofol and remifentanil, respectively. There was a significant difference between the propofol and remifentanil consumption per minute before and after the tourniquet pain in both groups: 5.61±1.67 and 14.58±6.62 mg/min propofol (p=0.001), and 4.79±1.69 and 7.86±1.55 mcg/min remifentanil (p=0.001), respectively. No patient had signs of local anesthetic toxicity. Conclusion: Low-dose sedo-analgesia can be used by a modified IVRA method in the management of tourniquet discomfort that may occur until the tourniquet pain develops.
