AimsTo evaluate (1) whether gambling problems predict overall trajectories of change in family or interpersonal adjustment and (2) whether annual measures of gambling problems predict time‐specific decreases in family or interpersonal adjustment, concurrently and prospectively.DesignThe Quinte Longitudinal Study (QLS) involved random‐digit dialling of telephone numbers around the city of Belleville, Canada to recruit ‘general population’ and ‘at‐risk’ groups (the latter oversampling people likely to develop problems). Five waves of assessment were conducted (2006–10). Latent Trajectory Modelling (LTM) estimated overall trajectories of family and interpersonal adjustment, which were predicted by gambling problems, and also estimated how time‐specific problems predicted deviations from these trajectories.SettingSoutheast Ontario, Canada.ParticipantsCommunity sample of Canadian adults (n = 4121).MeasurementsThe Problem Gambling Severity Index (PGSI) defined at‐risk gambling (ARG: PGSI 1–2) and moderate‐risk/problem gambling (MR/PG: PGSI 3+). Outcomes included: (1) family functioning, assessed using a seven‐point rating of overall functioning; (2) social support, assessed using items from the Non‐support subscale of the Personality Assessment Inventory; and (3) relationship satisfaction, measured by the Kansas Marital Satisfaction Scale.FindingsBaseline measures of ARG and MR/PG did not predict rates of change in trajectories of family or interpersonal adjustment. Rather, the annual measures of MR/PG predicted time‐specific decreases in family functioning (estimate: −0.11, P < 0.01), social support (estimate: −0.28, P < 0.01) and relationship satisfaction (estimate: −0.53, P < 0.01). ARG predicted concurrent levels of family functioning (estimate: −0.07, P < 0.01). There were time‐lagged effects of MR/PG on subsequent levels of family functioning (estimate: −0.12, P < 0.01) and social support (estimate: −0.24, P < 0.01).ConclusionsIn a longitudinal study of Canadian adults, moderate‐risk/problem gambling did not predict overall trajectories of family or interpersonal adjustment. Rather, the annual measures of moderate‐risk/problem gambling predicted time‐specific and concurrent decreases in all outcomes, and lower family functioning and social support across adjacent waves.

OBJECTIVE: To investigate the prevalence of therapeutic cannabis use within a general population sample of adults and to describe various characteristics associated with use.METHODS: Data were derived from the 2013 and 2014 CAMH Monitor Survey of adults in Ontario, Canada. This repeated cross-sectional survey employed a regionally stratified design and utilized computer-assisted telephone interviewing. Analyses were based on 401 respondents who reported using cannabis. RESULTS:The data indicated that 28.8% of those who used cannabis in the past year self-reported using cannabis for therapeutic purposes. Of therapeutic users, 15.2% reported having medical approval to use cannabis for therapeutic purposes. Cannabis use for therapeutic purposes was associated with more frequent use of cannabis, a moderate to high risk of problematic cannabis use, and a greater likelihood of using prescription opioids for medical purposes. There was little difference in cannabis use for therapeutic purposes according to sex, age, and marital status after adjusting for opioid use and problematic cannabis use. CONCLUSION:Findings suggest some potential negative consequences of cannabis use for therapeutic purposes; however, further research is needed to better understand the range and patterns of use and their corresponding vulnerabilities.KEY WORDS: Medical cannabis use; medical marijuana use; prescription opioids; problematic cannabis use La traduction du résumé se trouve à la fin de l'article.

Attention deficit hyperactivity disorder (ADHD) manifests as a behavioral pattern that leads to significant social, educational and occupational impairment. The main characteristics of the disorder are inattention, hyperactivity and impulsivity 1. The diagnosis of ADHD is clinical and is based on a persistent, early-onset clinical picture that manifests in different settings and leads to functional loss 2 . A failure in behavioral inhibition is believed to lead to the predominant symptoms of ADHD: hyperactivity, inattention, distraction and impulsivity 3 . The changes in the catecholaminergic pathways are believed also to involve the nucleus accumbens, with impairment of selective attention and the reward system 4 , which may be associated with a greater risk of substance use and dependence 5. The risk of dependence on alcohol or other substances is significantly higher in adolescents with attention-related difficulties than in healthy adolescents 6. Hence, because of the impulsivity and inattention associated with the condition, ADHD may be a risk factor for substance use disorder 5 , a set of cognitive, behavioral and physiological symptoms that cause occupational impairment as a result of the use of a particular substance. Substance dependence is a clinical condition at the more severe end of the substance use disorder spectrum. Even when occupationally incapacitated, dependent individuals continue to use substances. Drugs described as potentially causing dependence include nicotine, alcohol, cannabis, and cocaine and its derivatives ABSTRACTObjective: Attention deficit hyperactivity disorder (ADHD) is characterized by a persistent pattern of inattention or hyperactivity. This study aimed to investigate the relationship between ADHD and drug dependence. Methods: The presence and severity of ADHD and substance use were evaluated through questionnaires in 80 adult patients in therapeutic communities. Results: No difference in drug use or dependence prevalence between ADHD and non-ADHD patients was found. However, ADHD patients had lower ages on admission (p = 0.004) and at first contact with cocaine (p = 0.033). In ADHD patients, there was a negative correlation between the age at first use of cannabis and the subsequent severity of cannabis use (p = 0.017) and cocaine use (p = 0.033). Conclusions: Though there was no difference in prevalence of drug use among groups, results show that ADHD in patients in therapeutic communities may cause different addiction patterns, such as earlier use of cocaine and admission, and a more severe use of cocaine correlated to earlier contact with cannabis.Keywords: attention deficit disorder with hyperactivity; substance-related disorders; cocaine.
