Thing. A review of the literature devoted to the topical problem of maxillofacial surgery and surgical dentistry – methods of treating patients with mandibular fractures was carried out. The aim of the study is to review the materials of publications on the methods of treatment of patients with mandibular fractures. Methodology. The publications of domestic and foreign authors, considering methods of fixation of bone fragments in lower jaw fractures, have been studied. Results. Publications indicate that there are many treatments for mandibular fractures. Research results demonstrate the importance of bone stabilization for bone fusion. A common disadvantage of conservative and surgical methods of treatment is unstable fixation of bone fragments, which is the most common cause of delayed consolidation of a mandibular fracture. The presented studies show that bone osteosynthesis with metal plates is considered the most appropriate method for treating mandibular fractures. Conclusions. Splinting as a method of treating fractures of the lower jaw has a large number of disadvantages. A common disadvantage of methods for splinting the lower jaw is that these structures do not always provide anatomical reduction and stability for the entire period of consolidation or require mandatory intermaxillary fixation, the lower jaw is fixed in the central occlusion position, which is not a position of physiological rest, and leads to passive muscle tension. depriving them of active function. The main disadvantage of the methods of osteosynthesis using a suture and using wires is that it is difficult to achieve stable fixation of fragments using these techniques. Combined methods of fixation of bone fragments (use of a bone suture and dental splints) provide a stronger fixation of the fragments. In the past two decades, in the treatment of mandibular fractures, there has been an increase in the trend towards rigid or semi-rigid osteosynthesis using plates. The disadvantages of compression osteosynthesis is that significant compression can lead to ischemia and slow down the formation of callus. A common disadvantage of conservative and surgical methods of treatment is unstable fixation of bone fragments, which is the most common reason for their delayed consolidation. Bone osteosynthesis with metal plates is considered a more progressive method of treating mandibular fractures.

A special place in the methods of surgical treatment of bone fractures is occupied by transosseous osteosynthesis using external fixation devices of various designs.Objective: to analyze the lower jaw exoskeleton in finite element programs.Materials and research methods. During the research, 36 human lower jaws were involved from the museum of the Department of Oper-ative Surgery and Topographic Anatomy of the Volgograd State Medical University. They were scanned in 3D. All obtained images were loaded into a virtual scene reconstruction program. Fractures of the lower jaws in the corner area were modeled, an apparatus for external fixation of the lower jaw exoskeleton was installed, and the chewing load on the lower jaw was simulated. The place of application of the force was an area on a small fragment of a repositioned 3D model of the mandibular bone corresponding to the place of attachment of the masseter muscle. The evaluation of virtual studies was carried out according to the results of the stress-strain states of the bones and apparatus, the schedule of displacements of objects and the results of the analysis of the safety factor.Research results and discussion. In the course of evaluating the virtual placement of the mini-fixator wires of the apparatus for external fixation of the lower jaw exoskeleton, it was revealed that the main load is applied to the mini-fixator wires on a large fragment and the bone in the area of the wires. For the possibility of precise positioning of the osteofixers of the external fixation device, a device for the safe installation of the spokes of the lower jaw exoskeleton was also developed.Conclusion. Thus, on the basis of computer mathematical analysis, it can be argued that the developed design of the apparatus for external fixation of the lower jaw exoskeleton works under conditions of the maxillofacial region, performs reposition and fixation of bone fragments of the lower jaw under conditions of chewing functioning of the restored fracture of the lower jaw.
Summary. The paper presents a comparative analysis of the results of ultrasound examination of the masticatory muscles proper in patients with fractures of the branch and articular process of the lower jaw at various stages of treatment using the endoscopic method and traditional osteosynthesis.Research objective. To assess the morphological state of the masticatory muscles in patients with a fracture of the branch and articular process of the lower jaw using the endoscopic method and traditional osteosynthesis using ultrasound technologies.Resources and Methods. The results of ultrasound examination of the masticatory muscles in 82 patients with a fracture of the branch and articular process of the lower jaw, hospitalized in N.I. Pirogov State Clinical Hospital No. 1 for 2021, were analyzed.Research results. Based on the results of an ultrasound examination of patients aged 19 to 40 years with a mandibular fracture, it was found that the use of intraoral access using endoscopic technologies contributes to an earlier normalization of the morphological state of the masticatory muscles proper compared to traditional osteosynthesis.
