Background: Rheumatoid Arthritis (RA) is a chronic inflammatory disorder that impact daily life activities. The impact on work productivity is critical because of persistent work disability. Aim: To evaluate work productivity in RA patients compared to healthy controls, and to assess the impact of socio-demographic characteristics on work productivity. Methods: This was a case-control study conducted at the Rheumatology Unit of Baghdad Teaching Hospital, Medical City during the period from August 2020 to the end of March 2021. Seventy-two patients with RA were selected and compared with 72 healthy subjects as a control group. All patients were diagnosed according to ACR/EULAR 2010 RA classification criteria. The socio-demographic and clinical features, lifestyle practices, and disease activity of the patients and controls were all recorded during interviews. A standardized Arabic version of the Workplace Activity Limitation Scale (WALS) and the Job Limitations Questionnaire were used to assess the impact of RA on work productivity (WLQ-25). Results: The vast majority of patients were females (58.1%). Positive rheumatoid factor was reported in 94.4% of the patients. The patients showed a significantly lower WALS total score and higher WLQ-25 total score median (IQR) compared with controls. Conclusion: Active RA impairs the work productivity which was influenced by CDAI score and negatively associated with the use of DMARDs.

Introduction. Boron is a trace mineral that is supposed to be essential for human health. Organic plant based boron compounds are highly bioavailable for humans and can positively influence minerals such as calcium, phosphorus, magnesium and act in synergy with vitamin D, which are beneficial for osteoarticular health. Material and methods. We made an analysis of 126 bibliographic sources and mapped the Republic of Moldova to the content of boron in deep water and to the adults’ prevalence of rheumatoid arthritis and inflammatory polyarthropathy. Results. Boron is important for osteogenesis and its deficiency can lead to impaired growth and abnormal bones development. It supports bone health in postmenopausal women by reducing urinary loss of the minerals such as calcium, magnesium and phosphorus, which are essential for bone building. In countries where soil was depleted of boron and daily intake of this mineral was 1 mg or lower, the incidence of arthritis was between 20 and 70%. In Moldova, in the regions where boron concentration in deep drinking water were low (0.28 mg/L in well water and 0.51 mg/L in artesian well water) in Soroca in 2019, the prevalence with the rheumatoid arthritis and inflammatory polyarthropathy were high (51.6 per 10000 inhabitants) with one exception, UTA Gagauzia (35.1 per 10000 inhabitants), where we found a high boron concentration in deep drinking water (2.05 mg/L in well water and 2.2 mg/L in artesian well water) in 2015. Conclusions. Boron can affect bone metabolism. In regions where boron concentration in deep drinking water is low, the osteoarticular morbidity can be higher with the exception of UTA Gagauzia, where we suppose that dietary pattern can be a cause of these results. In the future we will study eating habits in this region and a clearer conclusion will be made.

Boron (B) is considered a prebiotic chemical element with a role in both the origin and evolution of life, as well as an essential micronutrient for some bacteria, plants, fungi, and algae. B has beneficial effects on the biological functions of humans and animals, such as reproduction, growth, calcium metabolism, bone formation, energy metabolism, immunity, and brain function. Naturally organic B (NOB) species may become promising novel prebiotic candidates. NOB-containing compounds have been shown to be essential for the symbiosis between organisms from different kingdoms. New insights into the key role of NOB species in the symbiosis between human/animal hosts and their microbiota will influence the use of natural B-based colon-targeting nutraceuticals. The mechanism of action (MoA) of NOB species is related to the B signaling molecule (autoinducer-2-borate (AI-2B)) as well as the fortification of the colonic mucus gel layer with NOB species from B-rich prebiotic diets. Both the microbiota and the colonic mucus gel layer can become NOB targets. This paper reviews the evidence supporting the essentiality of the NOB species in the symbiosis between the microbiota and the human/animal hosts, with the stated aim of highlighting the MoA and targets of these species.
