The Addition of Other Fecal Biomarkers Does Not Improve the Diagnostic Accuracy of Immunochemical Fecal Occult Blood Test Alone in a Colorrectal Cancer Screening Cohort

Abstract: Background: Screening with fecal occult blood test reduces colorectal cancer (CRC) incidence and mortality, and is currently implemented in most countries. However, around 40% of screening colonoscopies are normal. Thus, strategies to avoid these colonoscopies are highly necessary. Adding other fecal biomarkers, such as fecal calprotectin (FC), lactoferrin, and transferrin may be useful, but evidence is scarce.Aims: To evaluate the diagnostic accuracy of fecal occult blood immunochemical test (FIT), FC, and a … Show more

“…大肠癌病变组织周围一般有大量的炎性细 胞浸润, 能引起乳铁蛋白高表达, 该蛋白又能够跟随炎 症细胞漏至肠腔, 进一步在粪便中检测到. 虽然一些研 究发现粪便乳铁蛋白有助于检测CRC [52] , 也有其他研 究持相反结论 [53] .…”

“…Currently, the fecal immunochemical test (FIT) for occult blood testing is the most widely used standard noninvasive method for detecting precursor lesions and early stage CRC prior to colonoscopy [ 4 , 5 ]. Although FIT is cost-effective, it has low clinical potential due to its high false positive rate regarding colonoscopy outcome [ 6 ]: about 50% of theFIT(+) colonoscopies performed reveal no findings [ 7 ]. A low statistical specificity test not only increases the cost of the screening program but also unnecessarily exposes patients to an invasive and risky procedure such as colonoscopy.…”
