The Active Compounds and Therapeutic Target of Tripterygium wilfordii Hook. f. in Attenuating Proteinuria in Diabetic Nephropathy: A Review

Abstract: Tripterygium wilfordii Hook. f. (TWHF) is a traditional Chinese herbal medicine and widely used to treat diabetic kidney disease in China. Emerging evidences have revealed its ability to attenuate diabetic nephropathy (DN). Tripterygium wilfordii polyglycosides (TWPs), triptolide (TP), and celastrol are predominantly active compounds isolated from TWHF. The effects and molecular mechanisms of TWHF and its active compounds have been investigated in recent years. Currently, it is becoming clearer that the effect… Show more

“…Tripterygium Wilfordii Polyglycosides (TWPs) is the main compound of Tripterygium Wilfordii Hook. F. (TWHF) showing anti-fibrosis activity by reducing IL-1, IL-17, interferon-γ (IFN-γ), and TNF-α ( Liu et al, 2021 ). Previous studies show that Abelmoschus manihot reduces proteinuria and lessens kidney damage and tubulointerstitial fibrosis so that it improves kidney function.…”

“…DKD progression is normally considered irreversible, while Chinese herbal medicine gives us hope. Reliable evidences from RCTs show that Tangshen Formula and T. wilfordii Hook extract significantly reduce proteinuria and elevate eGFR compared with ARB or ACEI ( Ge et al, 2013 ; Li et al, 2015 ; Liu et al, 2021a ). Chinese herbal medicine normally targets multiple and synergetic targets to alleviate DKD due to multiple active compounds, including the improvement of metabolism regulation, the inhibition of oxidative stress and inflammation, the modulation of mitochondrial dysfunction, and the regulation of gut microbiota and related metabolism.…”

“…Modern pharmacology suggests that T. wilfordii exerts anti-tumor, anti-inflammatory, and immunosuppressive effects ( 5 , 6 ), and its main active ingredients are triptolide, celastrol, and total alkaloids of T. hypoglaucum ( 7 ). Its excellent curative effect is accompanied by some side effects; thus, scientists have conducted several studies to identify its toxic components and side effects and develop strategies to reduce them ( 8 , 9 ).…”
