The Action of L(-)-Dopa on Baroreflexes in Parkinsonism

Abstract: 1. The intra-arterial blood pressure responses to Valsalva's manoeuvre and postural change were investigated as indices of cardiovascular reflex activity in seventeen Parkinsonian patients on maximally tolerated doses of L( -)-dopa (levodopa) and repeated in nine patients on levodopa with an extracerebral decarboxylase inhibitor, L-alpha methyldopahydrazine. The results were compared with control pretreatment responses.2. There was significant impairment of the pressor response to Valsalva's manoeuvre on levod… Show more

“…With regard to possible iatrogenic effects of dopamine agonists, which have been shown to produce or worsen OH and cause cognitive deficits, we controlled for levodopa equivalent dosages and found no associated differences in OH severity or cognitive performance. 36,37 We also addressed potential confounds that would have otherwise affected interpretation of our neuropsychological test findings. As noted, we ensured that no participants had severe (hence distracting) symptoms during tilt, and while reliance on the subjective report has some inherent flaws, we are confident that the cognitive changes as observed in this study were not driven by state-based factors related to somatic stress.…”

“…6,7 Reflexive cardiovagal gain has been reported to be variably decreased in PD 8,9 and markedly decreased in PDϩOH. 10 Levodopa treatment of PD can affect reflexive cardiovagal function, 11 and decreased reflexive cardiovagal gain is more prominent in patients with long-standing PD, 12 who would be expected to be on levodopa. This study explored whether levodopa treatment is related to abnormal neurocirculatory function in PDϩOH.…”

“…The atrial tachyarrhythmia spontaneously reverted to sinus rhythm within 24 h after stopping bromocriptine. There has been no clinical or electrocardi6graphic (Reid et al, 1972). Animal investigations suggest that levodopa exerts its central hypotensive effect through formation of noradrenaline which interacts with noradrenergic receptors similar to \unable alpha-adrenoceptors (Henning & Rubenson, 1970).…”
