IntroductionThe full-endoscopic spinal surgery (FESS) procedure is widely accepted and welcomed in China. With the continuous development of minimally invasive surgery, the further expansion of indications and the greater diversity of techniques, spinal endoscopic surgery currently accounts for more than 10% of spinal surgery in China, ranking among the top in the world. However, the admission system and standardised training system for spinal endoscopic surgery are not perfect, which presents a challenge and disadvantage for novices.Methods and analysisExploratory mixed methods are applied for designing this study. First, we will collect questions from novices by allowing them to openly list their concerns to those who have completed FESS. These qualitative questions will be categorised using NVivo software. To produce the qualitative results, a questionnaire for the sequential two-round Delphi approach will be developed to identify the 20 most important questions from novices. This study is planned to be started at April 2021, and completed at March 2022.Ethics and disseminationThe Research Ethics Committee of Peking University Third Hospital provided a waiver for this Delphi protocol. We expect that the findings will be published in a clinical journal and presented at conferences. Furthermore, we hope that the results can contribute to answering the questions raised by novices of spinal endoscopy in the form of books and to improving the training system for spinal endoscopy surgery.

Depression could make the treatment outcome worse. However, up to now, no objective methods were developed to diagnose depression in hepatitis B virus (HBV)infected patients. Therefore, the dual metabolomic platforms were used here to identify potential biomarkers for diagnosing HBV-infected patients with depression (dHB). Methods: Both gas chromatography-mass spectrometry-based and nuclear magnetic resonance-based metabolomic platforms were used to conduct urine metabolic profiling of dHB subjects and HBV-infected patients without depression (HB). Orthogonal partial leastsquares discriminant analysis was used to identify the differential metabolites between dHB subjects and HB subjects, and the step-wise logistic regression analysis was used to identify potential biomarkers. Results: In total, 21 important metabolites responsible for distinguishing dHB subjects from HB subjects were identified. Meanwhile, seven potential biomarkers (α-ydroxyisobutyric acid, hippuric acid, azelaic acid, isobutyric acid, malonic acid, levulinic acid, and phenylacetylglycine) were viewed as potential biomarkers. The simplified biomarker panel consisting of these seven metabolites had an excellent diagnostic performance in discriminating dHB subjects from HB subjects. Moreover, this panel could yield a higher accuracy in separating dHB subjects from HB subjects than our previous panels (identified by single metabolomic platform) did. Conclusion: These results suggested that the dual metabolomic platforms could yield a better urinary biomarker panel for dHB subjects than any single metabolomic platform did, and our results could be helpful for developing an objective method in future to diagnose depression in HBV-infected patients.

Background: Contradictory conclusions of association between high-density lipoprotein cholesterol (HDL-C) levels and type 2 diabetes mellitus (T2DM) were reported in different populations. This study aimed to clarify the effect of HDL-C on the risk of T2DM incidence using a cohort lasting for 25-year follow-up.Methods: This study included 1462 community residents aged 55 or above, excluding subjects with diabetes at baseline. Incident cases of T2DM were ascertained by self-reported T2DM or fasting plasma glucose testing. The data were collected in nine surveys held between the years 1992 to 2017. Cox regression and the Fine-Gray model were applied to evaluate the relationship between HDL-C levels and the risk of T2DM incidence.Results: 120 participants developed new diabetes during follow-up period. There was a significant negative association between HDL-C levels and the risk of newly on-set T2DM. The incidences of T2DM decreased with the increasing quartile ranges of HDL-C levels, which were 12.60, 9.70, 5.38 and 5.22 per 1000 person-years, respectively. The adjusted hazard ratios (HRs) and 95% confidence intervals were 0.78 (0.50-1.23), 0.51 (0.29-0.90) and 0.47 (0.26-0.84) for subjects with baseline HDL-C levels in ranges of 1.15-1.39, 1.40-1.69 and ≥ 1.70 mmol/L, compared with ones in the range of < 1.15 mmol/L. Similar decreased HRs of T2DM for elevated HDL-C levels were observed in time-dependent Cox regression and other sensitivity analyses.Conclusion: Higher HDL-C levels were independently related to the lower risk of the newly-onset T2DM among the elderly in China.
