The 1 μg cosyntropin test in normal individuals: A reappraisal

Abstract: Background:The 1μg cosyntropin test has some advantages over the 250μg test as a test of adrenal function. One of the concerns regarding the 1 μg test includes stability of the cosyntropin when reconstituted and stored. Classically the 5th percentile responses to cosyntropin in normal individuals have been used to define a normal response. Recent studies have shown that these normative values should be determined for individual assays.Materials and Methods:We performed a 1μg cosyntropin test using reconstitute… Show more

“…Only dogs treated with Vetoryl ® were included in the study 1. Serum cortisol concentrations were measured before and 1 hour after IV administration of refrigerated 5 μg/kg ACTH.,2,3 Synthetic ACTH was reconstituted with 0.9% sodium chloride solution as per the manufacturer's instructions, and was refrigerated for no longer than 14 days 5. The study and consent form were approved by the Institutional Animal Care and Use Committee of the University of Pennsylvania.…”

“…Thus, for practical reasons, the low-dose test may be applicable to situations when corticotropin is in short supply or for other structural or economic reasons. Although dilution and storage of the lower concentrated solution may be regarded as critical, it is worth noting that diluted tetracosactide (in 0.9% saline at 1 g/mL) has been reported to be stable at 2-8°C for up to 60 days (60).…”

“…A previous study done in our institution showed that dilution and refrigeration of tetracosactide hexaacetate does not affect the validity of the testing. [ 8 ]…”
