ObjectivesThe aim of this study was to examine the well-being experiences of consultants working in paediatric critical care (PCC) settings in the UK during the COVID-19 pandemic.DesignQualitative design using individual interviews and thematic analysis.SettingPCC.ParticipantsEleven medical consultants working in PCC in a range of PCC settings/transport teams in the UK from nine units participated. Participants ranged in years of experience as a consultant from four to 23 years.MethodsA set of open semistructured questions were used to elicit information about participants’ experiences of workplace well-being. Interviews were audiorecorded and transcribed.FindingsThematic analysis identified six themes and data saturation was reached. These were as follows: (1) positive and negative impact of working during COVID-19, (2) job satisfaction and public scrutiny in the unique environment of PCC, (3) supporting the workforce through modified shift work, (4) perceptions of support and recognition offered from the hospital management, (5) successful coping strategies are personal and adaptive, and (6) importance of civility and good teamworkConclusionFindings show that consultants’ well-being is challenged in a number of ways and that the solutions to the problem of burn-out are multifaceted. Action is required from individual consultants, clinical teams, hospital management and national regulatory bodies. Our work corroborates the recent General Medical Council report highlighting doctors’ core needs for well-being: autonomy, belonging, competence. Burn-out is a long-term problem, requiring sustainable solutions. Future research needs to develop and evaluate the effectiveness of evidence-based interventions to improve consultants’ well-being. Trials of effectiveness need to present evidence that will persuade hospital management to invest in their consultants’ well-being within the economic context of reduced budgets and limited PCC workforce.

This paper proposes a new method to estimate behavioural curves from a stream of Near-Infra-Red (NIR) iris video frames. This method can be used in a Fitness For Duty system (FFD). The research focuses on determining the effect of external factors such as alcohol, drugs, and sleepiness on the Central Nervous System (CNS). The aim is to analyse how this behaviour is represented on iris and pupil movements and if it is possible to capture these changes with a standard NIR camera. The behaviour analysis showed essential differences in pupil and iris behaviour to classify the workers in "Fit" or "Unfit" conditions. The best results can distinguish subjects robustly under alcohol, drug consumption, and sleep conditions. The Multi-Layer-Perceptron and Gradient Boosted Machine reached the best results in all groups with an overall accuracy for Fit and Unfit classes of 74.0% and 75.5%, respectively. These results open a new application for iris capture devices.

Cannabis sativa is one of the oldest and most widely used plants in the world with a variety of industrial, medical, and nonmedical applications. Despite its long history, cannabis-derived products remain a source of controversy across the fields of medicine, law, and occupational safety and health. More favorable public attitudes about cannabis in the US have resulted in greater access to cannabis through legalization by states, leading to more consumption by workers. As more states adopt cannabis access laws, and as more workers choose to consume cannabis products, the implications for existing workplace policies, programs, and practices become more salient. Past workplace practices were grounded in a time when cannabis consumption was always viewed as problematic, considered a moral failing, and was universally illegal. Shifting cultural views and the changing legal status of cannabis indicate a need for research into the implications and challenges relating to cannabis and work. This commentary suggests research needs in the following areas: (a) data about industries and occupations where cannabis consumption among workers is most prevalent; (b) adverse health consequences of cannabis consumption among workers; (c) workplace supported recovery programs; (d) hazards to workers in the emerging cannabis industry; (e) relationship between cannabis consumption and occupational injuries; (f) ways to assess performance deficits and impairment from cannabis consumption; (g) consumption of synthetic cannabinoids to evade detection by drug testing; (h) cannabis consumption and its effect on occupational driving; and (i) ways to craft workplace policies and practices that take into consideration conflicting state and federal laws pertaining to cannabis. 1 | INTRODUCTION Cannabis sativa is one of the oldest and most widely used plants in the world. 1 Native to Asia, the different cultivars of C. sativa have been used for a wide variety of industrial, medical, and nonmedical uses for thousands of years. 2 Despite its long history, the use of cannabisderived products remains a source of controversy across the fields of medicine, law, and occupational safety and health. 3-6 More favorable public attitudes about cannabis in the United States have resulted in greater access to cannabis through legalization by states and led to more consumption.
