Testicular sperm extraction combined with intracytoplasmic sperm injection in the treatment of men with persistent azoospermia postchemotherapy

Abstract: Schizophrenia or schizoaffective disorders are often found in patients affected by DiGeorge/velo‐cardio‐facial syndrome (DGS/VCFS) as a result of hemizygosity of chromosome 22q11.2. We evaluated the UFD1L gene, mapping within the DGS/VCFS region, as a potential candidate for schizophrenia susceptibility. UFD1L encodes for the ubiquitin fusion degradation 1 protein, which is expressed in the medial telencephalon during mouse development. Using case control, simplex families (trios), and functional studies, we p… Show more

“…37 Moreover, reduced fertility after chemotherapy can be compensated for by improved assisted reproduction techniques. 38 Another very important aspect in the judgement of fertility of male patients after curative chemotherapy is the fact that even patients who were azoospermic for more than 4 years can still recover as observed in 2 patients in our series and by others. 36 Nevertheless, fertility status can be assessed by analyzing FSH and possibly inhibin B 39 serum levels.…”

“…17 But more importantly, reduced fertility due to low sperm count and poor sperm motility can be circumvented by assisted reproduction, in particular with intracytoplasmic sperm injection. 18 At present, cryopreservation of spermatozoa is an established option for sustaining the viability of sperm over a long period of time, allowing the sperm to be used for insemination at a later date. [19][20][21] Cryopreservation should be offered to all men at risk of infertility and should be done prior to treatment, and it must be emphasized and encouraged.…”

“…[22][23][24][25][26][27][28][29] At present, the optimum way to identify these pockets of sperm is to perform an extensive, surgical dissection of the seminiferous tubules (a testicular sperm extraction) (Level of evidence 2, Grade B recommendation). Large sections of the seminiferous tubules of the testis are examined with an operating microscope.…”

“…28 ICSI pregnancy rates using sperm from a testicular sperm extraction program are reported to be between 19% to 50%. 22,[25][26][27]30 The testicular sperm extraction procedure should be offered to all men with nonobstructive azoospermia but should only be undertaken in a centre where ICSI is available.…”
