“…More recently, simple sperm aspiration procedures such as testicular (TESA) 17 and percutaneous epididymal (PESA) 18 have been introduced. The introduction of Trucut needle testicular extraction 19 using a needle wider (14‐gauge) than traditional aspiration needles (19–21‐gauge) has allowed an increase in the numbers of sperm retrieved using non‐invasive techniques (mean of approximately 100,000 per retrieval). The question therefore arises that if a simple PESA or TESA are as effective in terms of retrieval rates, ICSI outcome 20 and patient satisfaction 21,22 as an open MESA with few complications 23 and neither require the expertise of a urological surgeon, why can they not be performed routinely within an IVF unit by suitably trained gynaecologists or perhaps nursing staff?…”
