Testicular (gonadal stromal) fibroma: Case report and review of the literature

Abstract: A 25-year-old man presented with complaint of a painless enlargement in his left testis. The solid, encapsulated, circumscribed and grayish-white testicular mass displayed the characteristics of testicular fibroma histologically. It was composed of acellular collagenized plaques and hypercellular areas of fibroblastic spindle cells. Immunohistochemically, the neoplastic cells were positive for vimentin and smooth muscle actin, but not for cytokeratin, S-100 protein, desmin, CD99/MIC2 (a protein expressed by Se… Show more

“…The average age of patients with TF is 31 (2). In our case, the patient denied any infection or trauma in the area of the testicle which is consistent with the data from Deveci et al (3). With our patient, there was no pain in the testicle, increase in tumor markers or pathological findings in the analysis of blood and urine, the same as with Pinieux et al, Renshaw et al, and Allen et al (7,5,8).…”

“…With our patient, there was no pain in the testicle, increase in tumor markers or pathological findings in the analysis of blood and urine, the same as with Pinieux et al, Renshaw et al, and Allen et al (7,5,8). Only a small number of cases of TF grew rapidly and were accompanied by pain (2,3). The tumor is most often localized in the central part of the testicular parenchyma having no contact with the tunic albuginea.…”

“…In Sertoli and Leydig cell tumors, features such as >3 or >5 mitoses per 10 HPF, size >5 cm, and an infiltrative border have been associated with an increased 12 In the series by Jones and colleagues, 1 case had up to 4 mitoses per 10 HPF and was without evidence of progression at either 2, 3, or 8 years after surgery (follow-up not specified for individual cases). In the current study, of the 2 men with 9 to 10 mitoses per 10 HPF, 1 died of other causes 5 years and 8 months later, and the other had no evidence of disease at 4 years and 10 months after surgery.…”

“…In the testis there is the rare possibility of tumors with exclusive fibrous and spindle cell components which do not involve the paratesticular tissue. Few cases have been reported of benign testicular gonadal stromal tumor with only spindle cells [8]. Few cases have been observed with atypic spindle tumor cells with nuclear pleiomorphism, numerous mitoses and blunt edged nuclei suggesting malignant tumors of smooth muscle origin.…”
