Background Testicular infarction/gangrene is usually caused by testicular torsion. Strangulated inguinal hernia complicated with testicular infarction is unusual in adults in contrast to many reported cases involving pediatric patients. Case presentation We report an unusual case of testicular gangrene in an elderly man presented with abandoned right obstructed inguinoscrotal hernia. He had right open hernioplasty with right orchidectomy. Conclusion Obstructed adult inguinoscrotal hernia potentially leading to testicular gangrene, despite being uncommon sequalae. The surgeon should always routinely examine the testis during surgery for complicated hernia to prevent missed gangrenous testis behind.

Testicular gangrene complicating strangulated groin hernia in an adult

Testicular infarction/gangrene is most commonly secondary to testicular torsion. Testicular infarction complicating a strangulated inguinal hernia has been well documented in pediatric literature but found to be rare in adults as it has only been reported in only 2 cases and none of which required orchidectomy for treatment. We report a rare case of strangulated inguinoscrotal hernia with testicular gangrene following a delayed presentation in a 25 year old boy with neglected groin hernia. The patient had orchidectomy in addition to primary bowel resection and anastomosis.

Acute scrotum is characterized by intense acute scrotal pain, which may be associated with other symptoms and signs such as abdominal pain, inflammation, and fever. Many pathologic conditions can present in this way, most which involve the scrotal contents. Nonscrotal conditions, however, can rarely present clinically only as acute scrotum: among them, renal colic, aneurysm rupture or other causes of retroperitoneal hemorrhage, primary abdominal or pelvic tumors and metastases, pancreatitis, pelvic inflammation, and muscle injuries. The pathophysiologic characteristics of the clinical presentation, clues for diagnosis, and imaging features of a series of nonscrotal lesions presenting clinically with acute scrotal pain are herein reported and illustrated. In patients presenting with acute scrotal symptoms and normal scrotal ultrasound findings, nonscrotal causes of acute scrotal pain should be considered in the differential diagnosis. Therefore, an ultrasound investigation of the abdomen, groin, and thighs is indicated.
