Terminology for Achilles tendon related disorders

Abstract: The terminology of Achilles tendon pathology has become inconsistent and confusing throughout the years. For proper research, assessment and treatment, a uniform and clear terminology is necessary. A new terminology is proposed; the definitions hereof encompass the anatomic location, symptoms, clinical findings and histopathology. It comprises the following definitions: Mid-portion Achilles tendinopathy: a clinical syndrome characterized by a combination of pain, swelling and impaired performance. It includes,… Show more

“…It generally presents with a gradual onset of pain following exercise, with localised tenderness and nodular thickening of the Achilles typically at the medial aspect of the mid tendon [36]. Insertional tendinopathy presents in athletes with a gradual onset of exercise-related localised pain, tenderness and swelling at the distal Achilles insertion.…”

“…It should be kept in mind that, the analysis of the experimental research with laboratory animals utilizing the components of the common calcanean tendon, like superficial digital flexor tendon or tendons of the medial or lateral head of the gastrocnemius, are applied to the human calcaneal tendon pathologies only by analogy and not by straightforward translation. Experimental study design should also include the evaluation of anatomical structures which are commonly involved in Achilles tendon related disorders in humans (van Dijk et al 2011). The Kager's (pre-Achilles) fat pad is a lipomatous structure located posterior to the talocalcaneal joint and anterior to the Achilles tendon in a standing man (Ly et al 2004, Theobald et al 2006.…”

“…In contrast, ultrasound shows paratenonitis as a dark hypoechogenic peritendinous halo due to the reduced echogenicity. Once the condition has become chronic, irregularities of the boundary zone of the paratenon can additionally occur and result in significantly increasing hypervascularization in power Doppler [44,45] ( • " Fig. 2).…”

“…In these cases, the bursa often exhibits histological degeneration of the bursal wall -at times with calcification or hypertrophy -and intraluminal fluid collection. In sonography, the fluid-filled bursa appears homogeneously hypoechogenic, while it appears hyperintense in T2-weighted MRI [45]. However, focal hyperintensity of the bursa is not to be primarily evaluated as pathological.…”
