RESUMOPacientes neurológicos terminais apresentam sintomas específicos e requerem cuidados paliativos específicos. Certas condições abruptas, como o Acidente Vascular Encefálico (AVE) e Traumatismo Crânio-Encefálico (TCE), levam a uma ruptura súbita com o estilo de vida anterior do paciente e dificuldade de compreensão da família e da equipe sobre a gravidade do quadro e prognóstico. Mesmo quando a terminalidade da vida não está evidente, diante de uma condição de saúde irreversível, sem possibilidade de resposta a um tratamento específico, a equipe médica deve considerar a introdução de cuidados paliativos, visando a promover conforto, qualidade de vida e dignidade ao paciente e seus familiares.Descritores: Cuidados paliativos. Acidente vascular cerebral. Traumatismo craniocerebrais. ABSTRACTTerminal neurological patients present specific symptoms and require specific palliative care. Some acute neurological conditions, such as Cerebrovascular Diseases (Stroke) and head trauma, lead to sudden rupture with the patient's previous lifestyle. It may be difficult for the family to understand the severity of the case, and for the assistants to estimate prognosis. Even when terminality is unclear, if there is an irreversible damage and no possibility of response to any specific treatment, the team must consider introducting palliative care, focusing on comfort, quality of life and patient's dignity.

Advance directives (ADs) are understood as the act of deciding what care the patient wants to receive in the period before death. Preserving the patient's autonomy by choosing his care guarantees human dignity during the process of dying. In Brazil, life expectancy and supportive technologies have increased, leading to growth of the number of terminally ill patients. However, there is still no legislation regulating ADs causing legal uncertainty in health professionals. Nursing professionals have the support of the Federal Nursing Council to respect the ADs, but, because it is an issue little explored, nursing professionals do not feel safe in the use of ADs, and changes in the curricula of the undergraduate courses in nursing are extremely needed, ensuring that patients have their wishes met during the dying process. Thus, this chapter deals with bioethical and legal issues involving ADs and nursing in the Brazilian context, proposing to deepen reflection and criticism on the issue and subsidies for decision-making.

Resumo: O objetivo do presente trabalho é compreender como os médicos do maior pronto-atendimento de uma metrópole brasileira orientam o cuidado aos pacientes graves com suspeita de morte encefálica e potenciais doadores de órgãos. Trata-se de uma etnografia, desenvolvida em um hospital de pronto-socorro, referência em trauma na América Latina, situado no hipercentro da cidade de Belo Horizonte, Minas Gerais. A instituição, de modo pioneiro no Brasil, conta com um setor específico para onde são encaminhados os pacientes com suspeita de morte encefálica. O trabalho de campo foi realizado durante nove meses, baseado em observações empíricas e entrevistas junto a 43 médicos plantonistas - 25 homens e 18 mulheres, entre 28 e 69 anos. A análise dos dados foi orientada pelo modelo de “signos, significados e ações”. À etnografia, emergiu o processo de cuidado médico ao paciente em suspeita de morte encefálica que contempla: terapia intensiva; realização do protocolo e comunicação do fato à família do paciente. Nesse último caso, a interlocução perpassa as controvérsias no conceito de morte encefálica, o contexto sociocultural e o contexto do pronto-atendimento. Ficou claro que esse processo de cuidado médico ultrapassa questões meramente normativas e adentra uma complexidade de elementos, sobretudo o papel do profissional como mediador de uma miríade de elementos e tensões imbricados. Entre a constatação e a comunicação da morte encefálica emergem percepções ambivalentes dos médicos e dos familiares. O estudo evidenciou que a tênue definição do que seja a vida e a morte tangencia toda a atuação médica, tendo implicações diretas no cuidado ao paciente/potencial doador e aos familiares.
