Temporary Extracorporeal Membrane Oxygenation in Patients with Refractory Postoperative Cardiogenic Shock-A Single Center Experience

Abstract: ECMO is a suitable technique for short-term treatment of refractory postoperative low cardiac output. Mortality rates are comparable to other cardiac assist devices, with approximately 30% of patients able to be discharged from hospital.

“…Based on the hypothesis of the beneficial effect of additional pulsatile flow, reduction of afterload, and better coronary flow, our policy is to use IABP routinely in patients undergoing ECMO (15). However, only 35% of the patients in our material had IABP prior to implantation of mechanical support which consequently was a deviation from our protocol.…”

“…1 Critical evaluation of pre-and perioperative risk factors before the termination of cardiopulmonary bypass (CPB) will usually provide an idea of the impending need for circulatory support. Should difficulty in weaning of CPB be imminent, plans should be made for the institution of pharmacologic and/or mechanical support (e.g., inotropic drugs, vasodilators, intraaortic balloon counterpulsation (IABP), ventricular assist devices).…”
