Temporal lobectomy for intractable epilepsy in patients over age 45 years

Abstract: Surgical management of uncontrolled focal epilepsy is most commonly carried out in young adults with good results, but there has been some doubt about the effectiveness of cortical resection in older individuals. We assessed the outcome of temporal lobectomy done after age 45 years in 20 patients with intractable epilepsy followed for more than 2 years after surgery. During a mean follow-up of 5 years, six patients (30%) were seizure-free following surgery and seven (35%) had greater than 90% reduction in seiz… Show more

“…34 A meticulous analyses of 126 studies published between 1991 and 2000 showed that a median of 70% of patients became seizure-free (range 33% to 93%), suggesting an improvement in the outcome in more recent studies. 35 Furthermore, selected patient populationssuch as patients with hippocampal sclerosis, 36 37 patients with complex febrile seizures, 38 or patients with foreign tissue lesions 39 40 -have significantly higher success rates, with 80-94% of patients becoming seizure-free.…”

“…In addition, there was no comparison with a younger population in some studies and the duration of follow-up after surgery was variable. McLachlan et al .,[62] compared seizure outcome 5 years after temporal lobectomy in 20 patients older than 45 years with that in 68 patients who were younger. The oldest patient was 60-years-old.…”
