Telerobotic-assisted laparoscopic hysterectomy for benign and oncologic pathologies: initial clinical experience with 30 patients

Abstract: Robotic surgery can be safely performed in gynecologic and gynecologic-oncologic surgery with no increase in complication rates. A significant advance is represented by the surgeon's ergonomic improvement.

“…In April 2005 the da Vinci  surgical system was approved by the Food and Drug Administration for use in gynaecological procedures and was shown by to have a role in gynaecological procedures (22)(23)(24)(25)(26). This technique has been shown to be feasible in gynaecological oncology for early endometrial cancer staging procedures (27)(28)(29)(30) as well as radical hysterectomies for early stage cervical tumours (31)(32)(33). Many of the barriers to use of traditional laparoscopy have been overcome using robotic surgical techniques.…”

“…Finally, the surgeon is seated in an ergonomically comfortable console which makes a prolonged case more tolerable. Robot-assisted laparoscopic hysterectomy has been shown to be safe and effective [7][8][9][10][11][12][13]. A study by Payne and Dauterive comparing TLH to RLH noted that the robotic cohort was associated with significantly less blood loss and decreased hospital stay, but longer operative time.…”

“…[6][7][8][9][10][11] A recent study by Payne et al comparing straight laparoscopic hysterectomy to RALH, noted that the robotic cohort was associated with significantly less blood loss, decreased hospital stay, but longer operative time. The intra-operative conversion rate to abdominal route from laparoscopic dropped from 9% to 4% when the robot assistance was introduced and there were no post-operative exploratory laparatomy in the robotic cohort as compared to 11% in the straight laparoscopic.…”
