Telerehabilitation versus traditional centre-based pulmonary rehabilitation for people with chronic respiratory disease: protocol for a randomised controlled trial

Abstract: BackgroundPulmonary rehabilitation is an effective therapeutic intervention for people with chronic respiratory disease. However, fewer than 5% of eligible individuals receive pulmonary rehabilitation on an annual basis, largely due to limited availability of services and difficulties associated with travel and transport. The Rehabilitation Exercise At Home (REAcH) study is an assessor-blinded, multi-centre, randomised controlled equivalence trial designed to compare the efficacy of home-based telerehabilitati… Show more

“…Of these, 7894 records were excluded at the title and abstract screening stage, and 97 full text records were assessed for eligibility ( Figure 1 ). A total of 84 of the 97 full text records were excluded for the following reasons: 26 did not measure smoking, nutrition, alcohol consumption, physical activity, and obesity outcomes; 23 did not use any form of video counseling intervention; 19 involved group video counseling and not individual video counseling [ 37 - 54 ]; 3 were protocol papers [ 55 - 57 ]; 8 were not randomized studies [ 58 - 65 ]; 2 studies were ongoing [ 32 , 66 ]; 1 was a conference abstract [ 67 ]; and 2 studies described a multicomponent intervention, and it was not possible to isolate the effect of real-time video counseling [ 68 , 69 ]. The remaining 13 eligible studies were included in the review.…”

“…The group who received the intervention showed greater PA immediately after the intervention than the usual care group, and the effect was still present 3 months after the intervention [ 36 ]. Another study about telerehabilitation in patients with pulmonary disease has been announced [ 37 ]. In this study, participants will undertake an 8-week group-based pulmonary rehabilitation program, either in-person in a center-based pulmonary rehabilitation program, or remotely from their homes via the internet [ 37 ].…”

“…Finally, the potential for spread of infectious respiratory pathogens from patient to patient is a concern in this population although there is currently no evidence that cross infection occurs in the group setting . More study is needed on home‐based PR and tele‐rehabilitation programmes . Finally, access to rehabilitation programmes is a problem worldwide and ways to maintain benefit must also be further evaluated.…”
