S troke is one of the leading causes of severe, long-term disability, 1 with a majority of stroke survivors requiring the assistance of a family caregiver.2 A family caregiver is defined in this context as a relative, partner, personal friend, or neighbor who provides assistance to an adult with a chronic or disabling condition such as stroke.3 Although family members may not consider themselves caregivers, this term is commonly used in the literature to represent family or informal (unpaid) caregivers.Family caregiver stress is commonly associated with longterm institutionalization of stroke survivors, resulting in significant costs to the healthcare system. [4][5][6][7][8] Family caregiver stress can also result in other negative outcomes for both survivors and their caregivers. 7 For example, caregiver stress has been shown to interfere with rehabilitation of the survivor 7 and can result in social isolation, declining health, and increased risk of mortality for the caregivers. 9,10 Depression is especially prevalent in stroke family caregivers, 7,11,12 with some studies reporting higher depression rates in the caregivers than in the survivors for whom they provide care. 11,13 Miller and colleagues 14 made recommendations for family caregiver education and support across inpatient, outpatient, and chronic care settings based on clinical practice guidelines and existing research. These recommendations involved (1) caregivers serving as integral members of interdisciplinary teams, (2) assessment of caregiver needs and concerns, (3) follow-up contacts and referrals, (4) counseling focused on problem solving and social support, (5) provision of strokerelated information, and (6) attention to the emotional and Abstract-Stroke is a leading cause of severe, long-term disability. Stroke family caregiver and dyad intervention studies have reported a variety of outcomes. A critical analysis of 17 caregiver intervention studies and 15 caregiver/stroke survivor dyad intervention studies was conducted to provide evidence-based recommendations for the implementation and future design of stroke family caregiver and dyad interventions. Objective: It is to determine interventions performed to develop competence of family caregivers of patients with stroke in coping with caregiving and what are they of these interventions. Method: 20 randomized controlled studies, one of them meta-analysis, were evaluated in accordance with inclusion criteria. All studies were systematized and examined. Results: Types of interventions made to the caregivers were consultancy via phone, e-mail, web aided training, home visits, and group education. The intervention provided to caregivers was found to be effective in 15 of 20 studies included in the study. In other five studies, there was no statistical difference in terms of a measured parameter; whereas, it was found useful for caregivers in at least one of the other measured parameters. Conclusion: The fact that interventions for strengthening caregivers were highly effective points out the requirement of determining cost effective methods in future studies.Key words: Stroke, family caregivers, caregivers competence, care burden, cope with stress, systematic review ÖZET Amaç: İnmeli hastaların, aile bakım vericilerinin, bakım vermede baş etme yeterliliklerini geliştirmek amacıyla yapılan girişimler ve bu girişimlerin neler olduğunun belirlenmesidir. Yöntem: Dâhil edilme kriterlerine uygun olarak, biri meta analiz olmak üzere, 20 randomize kontrollü çalışma değerlendirilmiştir. Tüm çalışmalar sistematize edilerek incelenmiştir. Bulgular: Bakım vericilere yapılan girişim türlerinin; telefonla danışmanlık, e-posta, web destekli eğitim, ev ziyaretleri ve grup eğitimi olduğu belirlenmiştir. Araştırma kapsamına alınan 20 araştırmanın, 15'inde bakım vericilere yapılan girişimin etkin olduğu belirlenmiştir. Diğer beş araştırmada ise, ölçülen bir parametrede istatistik açısından farklılık yok iken, ölçülen diğer parametrelerin en az birinde girişimin bakım verenler için faydalı olduğu belirlenmiştir. Interventions Associated With Reduced Loneliness and Social Isolation in Older Adults

ImportanceLoneliness and social isolation are public health concerns faced by older adults due to physical, cognitive, and psychosocial changes that develop with aging. Loneliness and social isolation are associated with increased morbidity and mortality.ObjectiveTo evaluate interventions, targeting older adults, associated with a reduction in loneliness and social isolation.Data SourcesOVID, CINAHL, CENTRAL, Embase, PsychINFO, Web of Science, and Scopus were searched from inception to March 2020.Study SelectionPeer-reviewed randomized clinical trials measuring loneliness and social isolation or support in adults aged 65 years or older. Only English language articles were included.Data Extraction and SynthesisTwo independent reviewers screened studies, extracted data, and assessed risk of bias. Random-effects models were performed to pool the overall effect size by intervention. Statistical heterogeneity was evaluated with the I2 statistic and by estimating prediction intervals. Data were analyzed from November 2021 to September 2022.Main Outcomes and MeasuresQuantitative measures of loneliness, social isolation, or social support based on an effect size of standardized mean differences.ResultsSeventy studies were included in the systematic review (8259 participants); 44 studies were included in the loneliness meta-analysis (33 in the community with 3535 participants; 11 in long-term care with 1057 participants), with participants’ ages ranging from 55 to 100 years. Study sizes ranged from 8 to 741 participants. Interventions included animal therapy, psychotherapy or cognitive behavioral therapy, multicomponent, counseling, exercise, music therapy, occupational therapy, reminiscence therapy, social interventions, and technological interventions. Most interventions had a small effect size. Animal therapy in long-term care, when accounting for studies with no active controls, had the largest effect size on loneliness reduction (−1.86; 95% CI, −3.14 to −0.59; I2 = 86%) followed by technological interventions (videoconferencing) in long-term care (−1.40; 95% CI, −2.37 to −0.44; I2 = 70%).Conclusions and RelevanceIn this study, animal therapy and technology in long-term care had large effect sizes, but also high heterogeneity, so the effect size’s magnitude should be interpreted with caution. The small number of studies per intervention limits conclusions on sources of heterogeneity. Overall quality of evidence was very low. Future studies should consider measures of social isolation in long-term care and identify the contextual components that are associated with a reduction in loneliness.
