BACKGROUND:While there are numerous benefits of smartphone use for physicians, little is known about the negative effects of using these devices in the context of patient care.OBJECTIVE:To assess resident and faculty smartphone use during inpatient attending rounds and its potential as a source of distraction during transfer of clinical information.DESIGN:Cross‐sectional survey.SETTING:University‐affiliated public teaching hospital.PARTICIPANTS:All housestaff and inpatient faculty in the departments of Medicine and Pediatrics.METHODS:Participants were asked about smartphone ownership, usage patterns during attending rounds, and whether team members had ever missed important data during rounds due to distraction from smartphones. Attendings were asked whether policies should be established for smartphone use during rounds.RESULTS:The overall response rate was 73%. Device ownership was prevalent (89% residents, 98% faculty), as was use of smartphones during inpatient rounds (57% residents, 28% attendings). According to self‐reports, smartphones were used during rounds for patient care (85% residents, 48% faculty), reading/responding to personal texts/e‐mails (37% residents, 12% faculty), and other non‐patient care uses (15% residents, 0% faculty). Nineteen percent of residents and 12% of attendings believed they had missed important information because of distraction from smartphones. Residents and faculty agreed that smartphones “can be a serious distraction during attending rounds,” and nearly 80% of faculty believed that smartphone policies should be established.CONCLUSIONS:Smartphone use during attending rounds is prevalent and can distract users during important information transfer. Objective: The purpose of this study was to evaluate the applicability of telediagnosis in oral medicine, through the transmission of clinical digital images by e-mail. Subjects and Methods: The sample included 60 consecutive patients who sought oral medicine services at the Federal University of Parana´, in the state of Parana´, located in southern Brazil. The clinical history and oral lesion images were recorded using clinical electronic charts and a digital camera, respectively, and sent by e-mail to two oral medicine consultants. The consultants provided a maximum of two clinical hypotheses for each case, which were compared with biopsy results that served as the gold standard. Results: In 31 of the 60 cases (51.7%), both consultants made the correct diagnosis; in 17 cases (28.3%), only one consultant made the correct diagnosis; and in 12 cases (20%), neither consultant made the correct diagnosis. Therefore, in 80% of cases, at least one consultant provided the correct diagnosis. The agreement between the first consultant and the gold standard was substantial (j = 0.669), and the agreement between the second consultant and the gold standard was fair (j = 0.574). Conclusions: The use of information technology can increase the accuracy of consultations in oral medicine. As expected, the participation of two remote experts increased the possibility of correct diagnosis.

Первый том монографии представляет собой обобщение научной работы в сфере истории медицины, проведенной в период 2006-2016 гг. Раскрыта динамика развития телемедицины в контексте эволюционных трансформаций телекоммуникационно-компьютерных технологий и систем здравоохранения. Представлена история телемедицины (как метода дистанционного предоставления медицинской помощи и услуг посредством телекоммуникаций) в государствах Австралии, Азии, Европы и Северной Америки в период 1850-1979 гг. Исследованы телемедицина как составная часть моделей и трансформаций систем здравоохранения, эволюция системотехни-ческих и организационных решений, деятельность академических и лечебно-профилактических учреждений, профессиональных сообществ по разработке, внедрению и оценке эффективности телемедицинских технологий. Впервые произведена периодизация развития телемедицины. Изу-чены основные этапы формирования методологии телемедицины; выявлены типовые характери-стики, особенности, тенденции и эффективность. Особое внимание уделено характеристике роли и вклада отдельных личностей и коллективов в развитие телемедицины. Процесс формирования телемедицины представлен как неотъемлемая составная часть систем здравоохранения в гло-бальной перспективе.Второй том монографии посвящен систематизации теории и методологии современной те-лемедицины. Впервые введено понятие «пропедевтика телемедицины», подразумевающее систе-матическое изложение основных понятийных, организационных, правовых, системотехнических и деонтологических вопросов. В этом разделе, также впервые, систематизирована методика фи-зикального, инструментального и лабораторного обследования пациента посредством телемеди-цинских приборов и средств. Методологически разделены клиническая и пациент-центрированная телемедицина. Для каждого направления приведены описания основных проце-дур, методов, форм применения. Специальные главы посвящены отдельным клиническим субдис-циплинам (телерадиологии, теледерматологии, телекардиологии, телеофтальмологии, телесто-матологии etc). Особое внимание уделено доказательности применения телемедицины: приведе-ны классификации, методы, примеры дизайнов исследований для оценки эффективности телеме-дицинской деятельности. Для клинических субдисциплин представлены результаты мета-анализов и крупных рандомизированных клинических испытаний, объективизирующие влияние телемедицины на организацию медицинской помощи, лечебно-диагностический процесс, его ре-зультаты.Для сотрудников медицинских и технических вузов, организаторов здравоохранения, прак-тических врачей, специалистов по биомедицинской инженерии, слушателей последипломного об-разования, интернов, аспирантов, магистров, студентов. © А.В.
