Purpose: to provide an update on the management of a Urology Department during the COVID-19 outbreak, suggesting strategies to optimize assistance to the patients, to implement telemedicine and triage protocols, to define pathways for hospital access, to reduce risk of contagious inside the hospital and to determine the role of residents during the pandemic. Materials and Methods: In May the 6 th 2020 we performed a review of the literature through online search engines (PubMed, Web of Science and Science Direct). We looked at recommendations provided by the EAU and ERUS regarding the management of urological patients during the COVID-19 pandemic. The main aspects of interest were: the definition of deferrable and non-deferrable procedures, Personal Protective Equipment (PPE) and hospital protocols for health care providers, triage, hospitalization and surgery, post-operative care training and residents' activity. A narrative summary of guidelines and current literature for each point of interest was performed. Conclusion: In the actual Covid-19 scenario, while the number of positive patients globally keep on rising, it is fundamental to embrace a new way to deliver healthcare and to overcome challenges of physical distancing and self-isolation. The use of appropriate PPE, definite pathways to access the hospital, the implementation of telemedicine protocols can represent effective strategies to carry on delivering healthcare.

Con el objetivo de detener la transmisión del coronavirus (declarado como pandemia por la Organización Mundial de la Salud [OMS]), y prevenir la propagación de la enfermedad COVID-19, el Gobierno de Colombia decretó un período de aislamiento social y confinamiento obligatorio en el territorio nacional a partir del 24 de marzo de 2020. El Instituto de Coloproctología (ICO) diseñó un programa de teleconsulta, junto con la entidad prestadora de los servicios de salud (EPS) SURA, apoyado en las tecnologías de la información disponibles. Se buscó mantener las actividades propias de la coloproctología y la fisioterapia del piso pélvico para garantizar a los pacientes la atención y el acceso ininterrumpido. Luego de 25 días, se programaron 626 consultas (coloproctología 62 % y fisioterapia del piso pélvico 38 %) con un porcentaje de ejecución del 94 % y una resolutividad del 78 %. Estas cifras demuestran que la telemedicina, al igual que otras actividades basadas en el teletrabajo, llegó para quedarse y trae consigo altos niveles de satisfacción para médicos, pacientes y entidades prestadoras de los servicios de salud. En este campo, también, el mundo nunca será el mismo.

The global pandemic caused by the new coronavirus, COVID 19, has disrupted the lives of most people on the planet. The magnitude of such disruption can only be compared to those caused by World War II. Experts suggest that in order to better evaluate this situation, it should be divided into two waves. The first wave being associated with health issues and the second one with economic issues. This article suggests that this global pandemic is fostering yet a third wave, which in the long run can be much more impactful in our lives than the first two. This third wave consists on accelerating the implantation process of the fourth industrial revolution. This article is divided into sections with the physical, digital and biological spheres of the fourth industrial revolution, as well as the dimensions of sustainability and other important considerations, in order to better demonstrate the emergence of a new era catalyzed by a virus.
