Telecare Provides Comparable Efficacy to Conventional Self-Monitored Blood Glucose in Patients with Type 2 Diabetes Titrating One Injection of Insulin Glulisine—the ELEONOR Study

Abstract: Patients adding one dose of glulisine at the meal with the highest postprandial plasma glucose excursion to titrated basal glargine achieved comparable improvements in glycemic control irrespective of traditional or telecare blood glucose monitoring.

“…The authors of this review agree with these statements and hypothesize the outcomes may indicate that the structured SMBG profile combined with educational tools not the telehealth RPM intervention, improved A1C. 37 Baseline A1C is an important consideration in planning telehealth RCTs. When study participants mean A1C is at or close to goal, it is not unrealistic to see a small IE.…”

“…Only 1 study titrating insulin doses defined specific fasting and postprandial glucose goals. 37 In some studies SMBG goals were individualized for the participant 41,42,46 and followed standard guidelines from the ADA 39,40 or both ADA and Veterans Administration.…”

“…The A1c levels of patients who used tele-care fell from 8.8% to 7% at the end of the 32nd week and the A1c levels of patients in the self blood glucose monitoring group fell from 8.9 % to 7.5 % at the end of the 32nd week. It was shown that the A1c levels of the tele-care group were reduced by 1.8 % and the A1c levels of the other group were reduced by 1.4% (P < 0.001) [28]. In a tele-medicine study on type 2 diabetics by Narayanan et al (2012), the A1c levels of diabetic patients were 9% higher than at first.…”
