Background: Home health service is the provision of health and care and follow-up services to patients in line with the recommendations of physicians, in the environment they live with their families, by the healthcare team in a way that meets their medical needs, including rehabilitation, physiotherapy and psychological treatment. This study was conducted to evaluate the medical conditions of pediatric patients receiving home health services.Methods: This descriptive study includes pediatric patients under the age of 18 who received home health services from Diyarbakır Pediatric Hospital in Turkey. The files of patients who applied to the home health unit between 2017 and 2020 and were actively followed up were reviewed retrospectively. The diagnoses, reasons for demand and medical needs of the patients were examined. Results:The mean age of 372 patients who received home health services was 10.9±4.3 years. 46.5% of the patients were diagnosed with epilepsy, 46.2% with mental retardation, 22.3% with cerebral palsy. Home health services are mostly requested for examination (49.2%), diaper (37.4%), medicine (14.8%) and formula (14.8%) reports. While 74.8% of girls are fully bedridden, 62.7% of boys are fully dependent (p<0.05). Most of the patients who are bedridden are provided with personal care by their families. 35.6% of bedridden patients and 29.6% of semi-dependent patients need medical care (p<0.05). While 63-67% of bedridden patients need rehabilitation, 38% of not bedridden patients need rehabilitation (p<0.05).Conclusions: Most of the patients receiving home health services are bedridden and have neurological diseases. As bed dependency increases, the need for medical care increases.

Giriş ve amaç: Gelişen tıbbi teknolojiler, kritik hastalığı olan bireylerin yaşam sürelerini uzatmıştır. Kendi başlarına gerçekleştiremedikleri eylemleri aygıtlar aracılığıyla gerçekleştiren çocuk olgu sayısı gün geçtikçe artmaktadır. Biz de bu noktadan yola çıkarak bu çalışmada bölgemizde çocuk acil servise başvuran özel bakım gerektiren çocukları incelemeyi amaçladık. Yöntem: Bu çalışma üçüncü basamak bir üniversite hastanesi çocuk acil servisinde yapılmış kesitsel tanımlayıcı bir çalışmadır. 01 Ocak 2017-31 Aralık 2017 arasında acil servise başvuran mekanik ventilatör, enteral beslenme kateteri ya da trakeostomi kanülü gibi teknolojik bir aygıta bağımlı olgular çalışmaya alınmıştır. Bulgular: Bu çalışmada 2106 acil servis başvurusu incelenmiştir. Çalışmaya daha önceden trakeostomi açılmış 9'u kız (%25), 27'si (%75) erkek olmak üzere toplam 36 olgu dahil edildi. Çalışmaya alınan olgular da teknolojik aygıt olarak 36'sında (%100) trakeostomi kanülü, 21'inde (%58.3) beraberinde ev tipi mekanik ventilatör ve 21'inde (%58.3) enterel beslenme katerteri vardı. Bu olguların trakeostomi açılma yaşı ortalamaları 25.7±11 (0.3-180) ay olarak saptandı. Trakeostomi açılmasındaki en sık neden uzamış entübasyon idi. Tekrar acil servis başvurularındaki en sık sebepler nörolojik durumlar, kontrol amaçlı olanlar ve enfeksiyon hastalıklarının tedavisi olarak saptandı. Acil servis başvurularının %8.7'si servis yatışı, %1.4'ü yoğun bakım yatışı ile sonuçlandı. Olguların 21'ine (%58.3) kardiyopulmoner resüsitasyon uygulandı, altı olguda (%16.7) eksitus saptandı. Sonuç: Trakeostomi durumu beraberinde ciddi bir mortalite ve morbidite getirmektedir. Bu durum hem birey hem de ona sağlık hizmeti veren ailesi açısından ciddi bir sorundur. Gerek koruyucu sağlık hizmetlerinin gelişimi ile böyle durumların ortaya çıkmasının engellenmesi gerek ise evde bakım hizmetlerinin ve palyatif bakım merkezlerinin gelişmesi ile bu iş yükü paylaşılıp daha kaliteli bir hizmet sunulacağı kanısındayız.

Introduction: The purpose of the present study was to examine the descriptive data regarding the children, their families, and living conditions in the records of home healthcare services provided for children between 01.04.2020 and 01.04.2021 at a children’s university hospital. Methods: The sample of this descriptive study consisted of the files of 170 pediatric patients who received service from Health Sciences University (SBU) Izmir Dr. Behçet Uz Pediatric Diseases and Surgery Training and Research Hospital Home Healthcare Services Unit between 01.04.2020 and 01.04.2021. The data were collected with the Data Record Form that was created by the researchers in line with the contents of the files kept for patients. The analysis of the data in this form was made in the SPSS package program 25.0 trial version. The data were described in terms of numbers, percentages, and averages, and the Chi-Square Significance Test was used in the comparisons made between the demographic data. The statistical significance level was considered as p<0.05. To conduct the study, permission was obtained from the Ethics Committee of S.B.U. Izmir Dr. Behçet Uz Pediatric Diseases and Surgery Training and Research Hospital. Results: The children who were included in the study were between the ages of 0 and 17 and the average age was 9.11±4.61. It was found that a total of 54.7% of the children were male, 97.6% were cared for by their mothers, 61.8% of the children whose files were examined had social security at the Social Security Institution (SSI), 51.8% were receiving service from the unit and were diagnosed with a neurological disease, and 58.8% were fully dependent in their daily living activities, 27.1% of them used mechanical ventilation, 37.6% used aspirator devices, 37.1% needed Oxygen (O2) support, 48.7% were fed orally, 27.7% were fed through the nasogastric route, and 23.6% were fed through Percutaneous endoscopic gastrostomy (PEG) and 4.2% of the participating children developed pressure sores. According to the records regarding the physical conditions of the places where children lived, 72.4% lived in apartments, 38.8% had a house that was heated by a heating system natural gas, 98.8% had good lighting in their homes, and 57.1% had alaturka toilet in their homes. Also, 64.8% of the children stayed in the living room of the house and 92.4% had good home hygiene. A statistically significant difference was detected between the income status of the children’s families and their social security (X2= 47.542 p=0.001, df=1). Conclusion: The factors such as the majority of patients registered to home healthcare services being cared for by their mothers, their high rates of dependency in daily living activities, some developing pressure sores, the need to be attached to a medical device, and varying home conditions in suitability for care show that patients have varying medical, psychological and social needs. For this reason, it would be beneficial to focus on pediatric studies in home healthcare services, organize.
