Techniques in occluding the aorta during endovascular repair of ruptured abdominal aortic aneurysms

Abstract: Among various methods to achieve rapid occlusion of the aorta during endovascular repair for ruptured abdominal aortic aneurysm, particular emphasis is placed on two techniques that have been incorporated into our endovascular repair practice. The sheath-over-balloon technique (the Loan SOB technique) facilitates hemodynamic stability by transfemoral endovascular placement of an aortic occlusion balloon catheter to the infrarenal abdominal aorta. The balloon-ahead-of-graft technique (the Hornsby BAG technique)… Show more

“…The positions of the renal arteries, aortic bifurcation, and common iliac bifurcation were identified on digital subtraction angiography with contrast injected using a power injector. 14 Once this was performed, the stent graft was deployed through the femoral artery while an occluder was passed into the common iliac artery on the contralateral side. A femorofemoral bypass was then performed to revascularize the contralateral lower limb.…”

“…Using local anaesthesia, this compliant balloon is inserted through the femoral artery, and under fluoroscopy, is positioned in the supracoeliac aorta 77. In haemodynamically unstable patients, the balloon is immediately inflated inside the aorta to provide control of the circulation and prevent blood loss through the ruptured aneurysm.…”
