Breast cancer is a heterogeneous disease, caused by the progressive accumulation of mutations and chromosomal abnormalities. Currently, breast cancer is considered the neoplasm with the highest incidence and mortality in women worldwide, so much so that every year in the world, one million breast cancers are discovered and around 400,000 women die from it. The search for diagnostic techniques that allow the detection of this pathology in an effective way has become essential and that is where mammography emerges, as a screening method, which has been shown to reduce mortality by detecting breast cancer early; however, in very dense breasts detection is difficult, so they have been modified, and thus generating new screening and diagnostic methods such as contrast-enhanced mammography, which is the newest and most promising imaging technique based on neovascularization of breast tumors in a similar way and may even be better than MRI.

La mamografía contrastada (CEDM, contrast-enhanced digital mammography) es una herramienta nueva que ha ido implementándose de forma creciente. Aparece como alternativa a la resonancia magnética (RM), y al igual que esta, tiene como principio el uso de contraste endovenoso para explorar la angiogénesis tumoral. Combina la imagen de mamografía convencional (Mx) con la técnica de sustracción con energía dual poscontraste, lo que resulta en un incremento en la detección de cáncer de mama, en un tiempo corto de estudio y a un bajo costo. Es un método prometedor en casos seleccionados y de fácil lectura, siendo útil principalmente en pacientes con diagnóstico de cáncer de mama para detectar lesiones adicionales y determinar el tamaño tumoral, ayudando en la planificación quirúrgica, así como también en la evaluación de la respuesta a la neoadyuvancia. También en el seguimiento de pacientes operadas, para caracterizar lesiones dudosas en Mx y ecografía, o como alternativa ante contraindicación de la RM. El objetivo de este trabajo es valorar la utilidad de la mamografía contrastada en la práctica diaria y determinar sus principales indicaciones. Repasamos con casos propios las utilidades y características del método.

Introduction.A standardized lexicon of morphology descriptors is essential for interpreting and reporting contrast enhanced mammography (CEM). This pictorial essay describes representative clinical cases using the first version of the BI-RADS ® lexicon for CEM. Material and Methods. Clinical cases, BI-RADS categories 1 to 6 with CEM examination, were eligible. CEM included low-energy (LE) and high-energy recombined (RC) images. Breast ultrasound (US) findings are also described. Biopsy histopathology of BI-RADS categories 4, 5, and 6 are reported. The first version of the BI-RADS ® lexicon was used for CEM reading, interpretation, and structured reporting. Results. We performed a detailed imaging morphologic analysis of all breast characteristics in clinical cases BI-RADS 1 to 6 using the first version of the BI-RADS ® lexicon for CEM. We describe normal, benign, or suspected malignant findings on US, LE and RC images of 8 clinical cases. Dense breasts were detected in 6 (75.0%) of the 8 cases. The presence of mass(es), radial scar, asymmetry, non-mass lesion, and microcalcifications are described. The histopathologic diagnosis in BI-RADS categories 4, 5, and 6 are shown. Conclusion. CEM increased the diagnostic accuracy in cases with nonconclusive results, suspicious malignant findings, or newly diagnosed breast cancer with the added value of using the first version of the BI-RADS ® lexicon for CEM.
