Tear trough deformity is one of the most common cosmetic concerns when it comes to facial rejuvenation in the Indian population. It is seen in a wide age group from 20 years of age to over 70 years of age. The tear trough deformity is restricted to the tear trough in most younger people but as one ages, the tear trough deformity includes the entire region under the eye and is better known as the infraorbital hollow. Hyaluronic acid fillers have been the fillers of choice in the treatment of the tear trough nonsurgically. Many treatment techniques have been described in literature. Both needles and cannulas have been used with equal efficacy. The author chooses to use both the needle and cannula in the same session to treat tear trough for a better cosmetic outcome with lesser chances of side effects.

Background The tear trough is the hollow concavity of the medial lower eyelid. Surgery can address tear trough deformities and reverse visible signs of periorbital aging. The previous methods of transconjunctival blepharoplasty with fat transposition were first described with subperiosteal placement of fat (Plast Reconstr Surg, 125, 2010,699, Aesthetic Surg J, 32, 2012, 426). This was followed by techniques with submuscular transposition of fat, which overcame certain difficulties associated with the subperiosteal methods (Clin Plast Surg, 20, 1993, 393, Arch Facial Plast Surg, 2, 2000, 16). Objectives We present a detailed description, evaluate the efficacy, safety, and advantages of transconjunctival blepharoplasty with fat pedicle transposition above the orbicularis muscle for lower eyelid rejuvenation and improvement of the tear trough deformity. Methods Forty‐one patients underwent lower eyelid blepharoplasty with fat transposition above the orbicularis muscle. Clinical and photographic documentation along with patient satisfaction ratings were evaluated for a minimum of 44 months postoperatively. Results Significant improvements of lower eyelid aesthetics and correction of tear trough abnormalities were observed in most patients. At 44 months postoperatively, surgical correction was maintained in all patients with a high satisfaction and long‐term survival. No contour irregularities or significant long‐term complications were detected in any of the patients. Conclusion Transconjunctival blepharoplasty with the fat pedicle transposition placed above the orbicularis muscle is a safe and effective technique for lower eyelid rejuvenation. Compared to previously described techniques of repositioning fat into the subperiosteal or submuscular plane, this technique of transposing fat above the orbicularis muscle is an alternative technique resulting in long‐term improvement of tear trough abnormalities with no major complications.

AimTo explore the efficacy of tear trough deformity treatment with the use of hyaluronic acid gel or autologous fat for soft tissue augmentation and fat repositioning via arcus marginalis release.Material and methodsSeventy-eight patients with the tear trough were divided into three groups. Class I has tear trough without bulging orbital fat or excess of the lower eyelid skin. Class II is associated with mild to moderate orbital fat bulging, without excess of the lower eyelid skin. Class III is associated with severe orbital fat bulging and excess of the lower eyelid skin. Class I or II was treated using hyaluronic acid gel or autologous fat injections. Class III was treated with fat repositioning via arcus marginalis release. The patients with a deep nasojugal groove of class III were treated with injecting autologous fat into the tear trough during fat repositioning lower blepharoplasty as a way of supplementing the volume added by the repositioned fat.ResultsSeventy-eight patients with tear trough deformity were confirmed from photographs taken before and after surgery. There were some complications, but all had complete resolution.ConclusionsPatients with mild to moderate peri-orbital volume loss without severe orbital fat bulging may be good candidates for hyaluronic acid filler or fat grafting alone. However, patients with more pronounced deformities, severe orbital fat bulging and excess of the lower eyelid skin are often better served by fat repositioning via arcus marginalis release and fat grafting.
