Objectives: Cardiopulmonary resuscitation (CPR) is a crucial yet weak link in the chain of survival for out-of-hospital cardiac arrest. We sought to understand the determinants of bystander CPR and the factors associated with successful training. Methods: For this systematic review, we searched 11 electronic databases, 1 trial registry and 9 scientific websites. We performed hand searches and contacted 6 content experts. We reviewed without restriction all communications pertaining to who should learn CPR, what should be taught, when to repeat training, where to give CPR instructions and why people lack the motivation to learn and perform CPR. We used standardized forms to review papers for inclusion, quality and data extraction. We grouped publications by category and classified recommendations using a standardized classification system that was based on level of evidence. Results: We reviewed 2409 articles and selected 411 for complete evaluation. We included 252 of the 411 papers in this systematic review. Differences in their study design precluded a meta-analysis. We classified 22 recommendations; those with the highest scores were 1) 9-1-1 dispatchassisted CPR instructions, 2) teaching CPR to family members of cardiac patients, 3) Braslow's selftraining video, 4) maximizing time spent using manikins and 5) teaching the concepts of ambiguity and diffusion of responsibility. Recommendations not supported by evidence include mass training events, pulse taking prior to CPR by laymen and CPR using chest compressions alone. Conclusion:We evaluated and classified the potential impact of interventions that have been proposed to improve bystander CPR rates. Our results may help communities design interventions to improve their bystander CPR rates. Disseminating Cardiopulmonary Resuscitation Training by Distributing 35 000 Personal Manikins Among School Children

Background-Because most cardiac arrests occur at home, widespread training is needed to increase the incidence of cardiopulmonary resuscitation (CPR) by lay persons. The aim of this study was to evaluate the effect of mass distribution of CPR instructional materials among schoolchildren. Methods and Results-We distributed 35 002 resuscitation manikins to pupils (12 to 14 years of age) at 806 primary schools. Using the enclosed 24-minute instructional DVD, they trained in CPR and subsequently used the kit to train family and friends (second tier). They completed a questionnaire on who had trained in CPR using the kit. Teachers also were asked to evaluate the project. The incidence of bystander CPR in out-of-hospital cardiac arrest in the months following the project was compared with the previous year. In total, 6947 questionnaires (19.8%) were returned. The 6947 kits had been used to train 17 140 from the second tier (mean, 2.5 persons per pupil; 95% confidence interval, 2.4 to 2.5). The teachers had used a mean of 64 minutes (95% confidence interval, 60 to 68) for preparation and a mean of 13 minutes (95% confidence interval, 11 to 15) to tidy up. Incidence of bystander CPR in the months after the project did not increase significantly compared with the previous year (25.0% versus 27.9%; Pϭ0.16). Conclusions-CPR training can be disseminated in a population by distributing personal resuscitation manikins among children in primary schools. The teachers felt able to easily facilitate CPR training. The incidence of bystander CPR did not increase significantly in the months following the project.

Bevezetés: Keringésmegállás esetén az azonnali laikus segítségnyújtás életmentő lehet. Célkitűzés: Célunk volt felmér-ni, hogy az általános iskolás korosztály hány éves kortól képes hatásosan végezni az újraélesztést. Módszer: A kutatás-ban 164 fő, 7-14 éves korú gyermek vett részt, amelynek során a résztvevők egy 45 perces újraélesztés-oktatáson vettek részt kis csoportokban (8-10 fő/csoport). Az oktatás végén az újraélesztési fantomon kétperces folyamatos újraélesztést kellett bemutatniuk, amelynek során a teljesítményt az "AMBU CPR Software" segítségével mértük. Eredmények: A mellkaskompressziók átlagos mélysége 44,07 ± 12,26 mm volt. A résztvevők 43,9%-a volt képes hatásos mellkaskompressziót kivitelezni. A lélegeztetés során befújt levegő mennyisége átlagosan 0,17 ± 0,31 liter volt. A résztvevők 12,8%-a volt képes a lélegeztetést hatásosan kivitelezni. A mellkaskompresszió mélysége (p<0,001) és a befújt levegő mennyisége (p<0,001) is szignifikáns összefüggésben volt a gyermek életkorával, testsúlyával, testmagasságával és BMI-értékével. Aim: Our aim was to determine at what age can schoolchildren perform correct cardiopulmonary resuscitation. Method: 164 schoolchildren (age 7-14) were involved in the study. A basic life support training consisted of 45 minutes education in small groups (8-10 children). They were tested during a 2-minute-long continuous cardiopulmonary resuscitation scenario using the "AMBU CPR Software". Results: Average depth of chest compression was 44.07 ± 12.6 mm. 43.9% of participants were able to do effective chest compressions. Average ventilation volume was 0.17 ± 0.31 liter. 12.8% of participants were able to ventilate effectively the patient. It was significant correlation between the chest compression depth (p<0.001) and ventilation (p<0.001) and the children's age, weight, height and BMI. Conclusions: Primary school children are able to learn cardiopulmonary resuscitation. The ability to do effective chest compressions and ventilation depended on the children's physical capability.
