TB vaccines: current status and future perspectives

Abstract: Vaccines against intracellular pathogens such as Mycobacterium tuberculosis need to induce strong cellular immune responses. Antigen discovery programs have exploited this and used proteome studies and T‐cell recognition in PPD‐positive individuals to select proteins and after testing for protective efficacy in animals the most promising proteins have been put together in fusion molecules. Three such fusion proteins are currently in clinical trials, the two most advanced have already passed phase I trials and … Show more

“…Both involving recombinant BCG, subunit vaccine and live attenuated Mycobacterium tuberculosis (Mtb) strains are currently under investigation. 2 Comparative genomics of the Mtb complex revealed that the attenuation of BCG is related to the loss of the protein secretion system ESX-1 due to deletion of the region of difference 1 (RD1) locus. 3 This segment is localized close to the origin of replication in all fully virulent members of the complex and harbors genes esxA (Rv3875), coding for the 6-kDa early secreted antigenic target (ESAT-6), and esxB (Rv3874), encoding the 10-kDa culture filtrate protein (CFP-10).…”

“…Available TB treatment involves daily administration of four oral antibiotics for a period of six months or more [7]. Due to the high percentage of side effects (like ototoxicity and nephrotoxicity) and the extended duration of treatment results in low patient adherence [8]. History of tubercular chemotherapy started with discovery of streptomycin in 1944 [9].…”

“…Vaccine candidates currently in clinical trials include improved recombinant BCG vaccines, virus-based recombinant vaccines, and subunit vaccines comprised of dominant secreted antigens (1,32). Secreted proteins, regularly described as culture filtrate proteins (CFP), are the main targets of the T-cell response in mice, both at the height of infection and in a state of memory immunity, as well as in humans with active TB (1,4,5,7,23). Immunization with these antigens in the presence of different adjuvants provided protection in mice challenged with M. tuberculosis, and protection was mediated by gamma interferon (IFN-␥)-producing CD4 ϩ cells (29,38).…”
