Tarsal coalition in paediatric patients

Abstract: Tarsal coalition is due to failure of segmentation between two or more foot bones during embryological development at a site where the joint cleft fails to develop. Depending on the nature of the tissue connecting the bones, the abnormality is a syndesmosis, synchondrosis, or synostosis. Although the coalition exists at birth, synostosis usually develops only late during growth. Talo-calcaneal and calcaneo-navicular coalitions account for over 90% of all cases of tarsal coalition. The joint at the site of the … Show more

“…The mean age at treatment was 11.8 years (9-17): 11.6 (9-17) for the nonoperative group and 12.2 (10)(11)(12)(13)(14)(15) for the operative group There were no correlations between AOFA-AHS at baseline and sex, age, bilaterality, radiografic features. In all cases, the area of the coalition involved less than 50% of the subtalar joint and no radiographic OA was observed.…”

“…Recommended treatment includes manipulation, continuous or intermittent casting and orthosis, while surgery is generally reserved to those cases in which nonoperative treatment fails. (1,(13)(14)(15)39) Concerning the nonoperative treatment, it can relieve pain and improve function in 25%-68% of cases. (18,(40)(41)(42)(43)(44) Most authors suggested that surgery should be performed on patients whose symptoms were not relieved by conservative treatment.…”

“…29 Many authors agree that conservative treatment must be initially attempted, while surgery should be reserved when conservative treatment fails. 2,12,24,28,29 Historically, subtalar or triple arthrodesis has been recommended for pain relief. 9,35 More recently, some authors reported good results following bar resection, possibly associated with interposition of various tissues.…”
