Target attainment of cefotaxime in critically ill children with meningococcal septic shock as a model for cefotaxime dosing in severe pediatric sepsis

Abstract: Reduced target attainment of β-lactam antibiotics is reported in critically ill patients. However, as target attainment of cefotaxime in severely ill pediatric sepsis patients may differ from adults due to age-related variation in pharmacokinetics, we aimed to assess target attainment of cefotaxime in this pilot study using meningococcal septic shock patients as a model for severe sepsis. Secondary analysis of prospectively collected data from a randomized controlled trial. Children with meningococcal septic s… Show more

“…Béranger et al and Hartman et al defined higher targets of 100% f T> MIC and 100% f T> 4x MIC. They only found satisfactory target achievement against strains with low MIC ( ≤ 0.5 mg/L) with usual dose regimens of 100–150 mg/kg/day and suggested applying continuous infusion in septic children ( 22 , 23 ).…”

“…eGFR, serum creatinin, albumin, C-reactive protein, transaminases) as covariate were limited [21,25,26,28,51,52,55,57]. Of the 48 studies, only 5 PK models have incorporated eGFR in their estimations for drug Cl [21,26,51,55,57] and only 3 additional studies found a significant relation between serum creatinine and drug concentration [29,45,60]. Other studies have either not investigated this relationship or found the addition of creatinine clearance to be non-significant.…”

“…While three studies were found reporting cefotaxime PK in critically ill children [27][28][29], most children in the study by Von Hattingberg et al were neonates, with only two patients > 1 month of age included in the PK analysis [27]. Doses used varied from 100 to 300 mg/kg/day in 3-4 daily doses, with patients > 50 kg receiving adult doses of three daily doses of 1000 mg in the study by Béranger et al [28].…”

“…Additionally, an MIC non-species related breakpoint of 1 mg/L is commonly used for the treatment of a susceptible pathogen, with an MIC of >2 mg/L indicating a resistant pathogen 13 . Current cefotaxime dosing regimens of 50 mg/kg every 6 h, with a maximum dose of 2 g (a total daily dose of up to 8 g), are commonly used for critically ill paediatric patients (>one month old of life) [14][15][16] .…”
