Tandem high-dose chemotherapy strategy as first-line treatment of primary disseminated multifocal Ewing sarcomas in children, adolescents and young adults

Abstract: The prognosis of primary disseminated multifocal metastatic Ewing's sarcoma (PDMES) is poor even if a slight improvement has been achieved with high-dose alkylating agent-containing chemotherapy. To enhance treatment efficacy, we assessed the feasibility, safety and efficacy of a tandem high-dose chemotherapy (HDC) regimen. In a single institution, patients with PDMES received six courses of vincristine/ifosfamide/doxorubicin/etoposide induction therapy, followed by high-dose thiotepa, and then melphalan-busul… Show more

“…In landmark as well as in recent studies, improvement of local therapy has proven crucial in the treatment of advanced Ewing sarcoma patients [11, 20, 24], whereas the role of high-dose chemotherapy regimen with autologous stem cell transplantation in the treatment of advanced Ewing sarcoma patients remains subject to an ongoing discussion [4, 7, 10, 1517, 2535]. The conflicting results of various studies including our previous analyses [6, 20] in contrast to the results of Meyers et al [21] may be due to variation in intensity of local therapy.…”

“…This definition applies to either single cytostatic agents or drug combinations [ 2 ]. The prognostic value of high RDI in cancer survivorship has been discussed in several studies [ 3 8 ] and for many cancer types; however, this debate has never reached consensus [ 9 , 10 ].…”

“…To date, the use of intravenous busulfan in high-risk ES was reported for limited cohorts of patients [27,28]. Since 2007, the Rizzoli Orthopaedic Institute, Bologna, has started the transplant activity for high-risk ES patients by using intravenous busulfan in the BU-MEL conditioning regimen before ASCT.…”
