Background Immunotherapy is a firmly established pillar in the treatment of cancer, alongside the traditional approaches of surgery, radiotherapy, and chemotherapy. Like every treatment, also cancer immunotherapy causes a diverse spectrum of side effects, collectively referred to as immune-related adverse events. Objective This review will examine the main forms of immunotherapy, the proposed mechanism(s) of action, and the incidence of thyroid dysfunctions. Methods A comprehensive MEDLINE search was performed for articles published up to March 30, 2017. Results Following the pioneering efforts with administration of cytokines such as IL-2 and IFN-g, which caused a broad spectrum of thyroid dysfunctions (ranging in incidence from 1 to 50%), current cancer immunotherapy strategies comprise immune checkpoint inhibitors, oncolytic viruses, adoptive T-cell transfer, and cancer vaccines. Oncolytic viruses, adoptive T-cell transfer, and cancer vaccines cause thyroid dysfunctions only rarely. In contrast, immune checkpoint blockers (such as anti-CTLA-4, anti-PD-1, anti-PD-L1) are associated with a high risk of thyroid autoimmunity. This risk is highest for anti-PD-1 and increases further when a combination of checkpoint inhibitors is used. Conclusions Cancer patients treated with monoclonal antibodies that block immune checkpoint inhibitors are at risk of developing thyroid dysfunctions. Their thyroid status should be assessed at baseline and periodically after initiation of the immunotherapy.

Talimogen laherparepwek (T-VEC) to pierwsza (ostatnio zarejestrowana w Unii Europejskiej) immunoterapia wirusem onkolitycznym, która wykazała korzyść terapeutyczną w leczeniu czerniaka skóry w badaniu klinicznym III fazy, powodując większy odsetek trwałej odpowiedzi na leczenie oraz medianę przeżyć całkowitych, w szczególności w grupie nieleczonych wcześniej chorych w nieresekcyjnym stadium IIIB, IIIC lub IVM1a. T-VEC był dobrze tolerowany i jest obecnie badany w leczeniu skojarzonym z inhibitorami punktów kontrolnych układu immunologicznego oraz terapii neoadiuwantowej. W pracy omówiono aktualne dane dotyczące skuteczności i bezpieczeństwa tego leku w leczeniu zaawansowanego czerniaka. Talimogene laherparepvec (T-VEC), review of a new therapy of cutaneous melanoma with genetically modified oncolytic virusTalimogene laherparepvec (T-VEC) is the first (approved recently in the European Union) oncolytic immunotherapy which demonstrated therapeutic benefit for cutaneous melanoma in a phase III clinical trial. This resulted in a higher that previously obtainable response rate and median overall survival, particularly in untreated patients or those with stage IIIB, IIIC, or IVM1a disease. T-VEC was well tolerated and is currently studied in combined therapy with immunological checkpoints and in a neoadjuvant setting. logic practice. 3 However, while historically syphilology was a major focus of dermatology, it is unclear who primarily diagnoses syphilis in the modern era.Methods | We performed a retrospective review of cases of syphilis reported to the Rhode Island Department of Health between 2010 and 2014. This study was deemed exempt by the Rhode Island Department of Health institutional board review. We collected the following data: sex, age, race, sexual orientation, human immunodeficiency virus (HIV) status, chief complaint or reason for testing, clinician type, and stage of syphilis. An internet search for the physician's name was used to identify the specialty. Cases of reinfection were excluded.Results | There were 364 cases of syphilis that met study criteria. Most patients were white (229 [62.9%]), and the average age was 35.8 years. Most patients were male (335 [92.0%]), particularly men who have sex with men (MSM) (272 [74.7%]). Most patients were HIV-negative (210 [57.7%]), although 107 patients were positive (29.3%) and 47 statuses were unknown (12.9%). Of the 283 MSM, 102 were HIV-positive (36.0%).The most frequently seen clinician was in the primary care or community health care setting (129 [35.4%]), followed by infectious disease specialists (93 [25.5%]), urgent or emergency care (66 [18.1%]), and clinics for sexually transmitted infections (STI) (41 [11.3%]). Nine patients (2.5%) were diagnosed by dermatologists: 7 with an erythematous eruption and 2 others with a genital lesion. The 2 most common reasons for testing were erythematous eruption and routine screening.or offering screening to asymptomatic individuals who are at risk, dermatologists can substantially contribute to the control and elimination of syphilis.
