Takayasu arteritis associated with ulcerative colitis and optic neuritis: first case in Korea

Abstract: Takayasu arteritis (TA) is a chronic vasculitis that affects the aortic arch and its primary branches. Ulcerative colitis (UC) is an inflammatory bowel disease of unknown etiology. Patients diagnosed with both TA and UC have rarely been reported. The pathogenesis of TA and UC is uncertain, but cell-mediated mechanisms play an important role in both diseases, and a genetic factor is thought to have an effect on the coincidence of these two diseases. We herein report a 38-year-old female with TA who had a histor… Show more

“…The cases about coexistence of TA and UC are rarely reported over the world and most of them are Japanese. In Korea, there are only a few cases, including the case reported by Pyo et al [8]. To the patient with both TA and UC, we used PET/CT for assessing not only the disease activity but also treatment response, which is the unique point of our report.…”

“…It has been well known that immunologic abnormalities are present in TA and UC [8]. In TA, natural killer-cell and T cells infiltrate the aorta, and the levels of several cytokines such as TNF, interleukin (IL)-6, or IL-18 increase.…”

“…In this setting, a work-up with cranial and orbital MRI, lumbar puncture and CSF studies including oligoclonal band and IgG index, and if needed further investigations for spinal MRI and neuromyelitis optica (NMO) IgG should be performed 10,11 . ON has been reported in the past with other immune mediated disorders like Behcet's disease, Sjogren's syndrome, Takayasu arteritis and systemic lupus erythematosus 9,[11][12][13][14] . Those cases might support a probable relationship between autoimmune and autoinflamatory disorders and ON.…”

“…Aortic root dilatation may lead to congestive cardiac failure [23]. A number of cases of ulcerative colitis associated with TA have been documented, especially in Japan [24][25][26][27][28]. An association with uveitis has been reported in some series from Mexico, USA and South Africa [16,29,30].…”
