Systemic Reactions to Topically Applied Drugs

Abstract: Topical drugs can be absorbed percutaneously in sufficient amounts to cause untoward systemic effects. Generally speaking, infants and young children appears to be more vulnerable than adults. It has been shown that the prolonged and extensive use of topical corticosteroids can induce iatrogenic hypercorticism and that they may also impair the patients ability to cope with stress. Furthermore, the relatively limited use, areawise and timewise, of agents such as phenol, resorcinol, salicylic acid, hexachlorophe… Show more

“…The solution used in this study comprised 4.5 g phenol, 10 g resorcinol, 0.3 g basic fuchsin, 5 ml acetone, 9.4 ml of 80% ethanol, and 85.6 ml of distilled water, of which the components of this solution have antifungal (carbol-fuchsin), antibacterial (ethanol and resorcinol), and acidic (acetone) eVects. Although some studies have suggested the possibility that some ingredients are toxic (e.g., phenol, acetone, and resorcinol), they comprise less than 5-10% of the total solution and castellani solution has been used only topically [18]. Moreover, in a recent animal study, topical application of castellani solution to the round window membrane in the middle ear cavity did not have any eVect on the outer hair cell function [19].…”

“…Phenol (hydroxybenzene, carbolic acid) has been used as an antiseptic for the sterilization of ulcers and burns. 55 Phenol is a potent CNS and respiratory system stimulant as well as a protoplasmic poison and can cause renal failure by direct damage to the kidney. The mechanism for this is the uncoupling of oxidative phosphorylation.…”

“…A 40% peeling resorcinol paste applied daily for 3 weeks produced dizziness, pallor, cold sweat, tremors, and collapse on final application. [318] Resorcinol has an anti-thyroid activity. Hence, continuous application can cause myxedema.…”
