Systemic polyarteritis nodosa as the initial manifestation of a gastric adenocarcinoma

Abstract: We report a case of systemic polyarteritis nodosa (PAN) leading to the discovery of an as yet asymptomatic, surgically curable gastric adenocarcinoma. PAN is rarely associated with malignancies and in such cases these are more often malignant haematological diseases than solid neoplasms. The immunopathological findings, the temporal relationship between both conditions, and the spontaneous resolution of vasculitis after tumour removal suggest a paraneoplastic origin of the systemic angitis.

“…The use of the Watts' algorithm in our patient resulted in a diagnosis of MPA. To the best of our knowledge, the concurrent presence of PAN and gastric cancer has been reported in only two patients [12,13], but no case of AAV associated with gastric cancer has been reported to date. Gastric cancer is thought to be rarely associated with any type of vasculitis.…”

“…The cause of the atrophy was not apparent with neurologic diagnostic modalities, and therefore the case was assumed to be due to a paraneoplastic manifestation [9]. There is also a case report of systemic polyarteritis nodosa leading to the discovery of an asymptomatic, surgically curable gastric adenocarcinoma, and a diagnosis of paraneoplastic systemic angiitis was given [10].…”
