Systemic Lupus Erythematosus: Definitions, Contexts, Conflicts, Enigmas

Abstract: Systemic lupus erythematosus (SLE) is an inadequately defined syndrome. Etiology and pathogenesis remain largely unknown. SLE is on the other hand a seminal syndrome that has challenged immunologists, biologists, genetics, and clinicians to solve its nature. The syndrome is characterized by multiple, etiologically unlinked manifestations. Unexpectedly, they seem to occur in different stochastically linked clusters, although single gene defects may promote a smaller spectrum of symptoms/criteria typical for SLE… Show more

“…Systemic lupus erythematosus (SLE) is a chronic autoimmune disease with several clinical manifestations and immunological alteration [1]. The interaction of sex, environmental factors, and hormonal status, including especially the hypothalamo-pituitary-adrenal axis (HPA) alteration, contribute to the development and existing of the disease.…”

“…The interaction of sex, environmental factors, and hormonal status, including especially the hypothalamo-pituitary-adrenal axis (HPA) alteration, contribute to the development and existing of the disease. SLE is characterised by the loss of immune tolerance, aberrations of communication, differentiation, and activation of immune cells and pathways [1,2]. Besides regulating metabolism, blood pressure, inflammation, and many other processes, changes in the cortisol level influence the cognitive function, the behaviour, and the reaction to stress [14].…”

“…In the introduction to the revised SLICC SLE classification criteria it is stated “ To ensure that there is a consistent definition of SLE for the purposes of research and surveillance, classification criteria for SLE are needed ” ( 2 ). This statement indicates that the ACR or SLICC classification criteria are valid as reliable approaches to define SLE, even though they do not define SLE as a homogenous disease since the classification criteria by definition provides hundreds of clinical phenotypes [discussed in ( 3 )]. Figure 1 principally demonstrates the clinical phenotype variability problem.…”

“…Systemic lupus erythematosus (SLE), a chronic inflammatory disease characterized by multiple immunologic abnormalities, can damage many organs [1]. The prevalence rates of SLE are about 17-48/100,000 population worldwide, and females are 3-6 times more frequently affected than males.…”
