Systemic Embolism and Septic Shock Complicated Left Atrial Myxoma: Case Report

Abstract: Myxoma is the most common primary tumor of the heart. The rarity of infected cardiac myxomas leads to numerous diagnostic and therapeutic difficulties. We present a case of infected left atrial myxoma caused by methicillin-sensible Staphylococcus aureus in a 48-year-old woman complicated by systemic embolism and septic shock.

“…With regards to management, surgical excision of the infected myxoma and intravenous antibiotics appear to be therapeutic [ 6 ]. Some reports note an urgency for surgical removal in order to prevent catastrophic embolization, as infected myxomas are two- to threefold more likely to embolize as compared to noninfected [ 12 ]. Other reports however opted to pretreat with antibiotics for several weeks prior to surgical removal [ 3 ].…”

“…Including our case, 47 of 58 cases (81.0%) were caused by gram-positive cocci, and 14 of them were caused by S. aureus . Five cases combined with DIC have been reported, and all five cases were infected by S. aureus [ 4 , 5 , 8 , 9 ]. DIC causes a haemostatic disorder and micro-thromboembolisms in small systemic arteries, which leads to multiple organ failure and creates a much higher operative risk.…”
