Mobilization of nurses’ agency across healthcare sectors is needed to counter challenges associated with older adults’ transitions between hospital and primary care. Based on Cultural Historical Activity theory and the Change Laboratory method, we developed a learning intervention with 16 nurses. The aim was to foster the nurses’ transformative agency to improve care. Video-recording of nine learning sessions were transcribed and analyzed. Results demonstrated that shared transformative agency exhibited as an emergent phenomenon crossing sectoral boundaries as a prerequisite for change in transitional care. The nurses progressed from acting as individuals criticizing the current conditions to collectively forming a vision around a transitional care model. This was nurtured through the nurses’ negotiations which included a recognition of sharing similar challenges deriving from the healthcare organization and related financial restrictions, and conflicting healthcare and nursing ideals across healthcare sectors. The evolution of transformative agency was grounded in a professional nursing identity.

“…Apesar de esse processo ocorrer durante a internação, é no momento da alta hospitalar que acontece a transição do paciente para os demais serviços da Rede de Atenção à Saúde (RAS) e quando ele deixa de receber o cuidado da equipe de Enfermagem hospitalar e passa a fazer o acompanhamento na atenção primária à saúde (APS). 5,6 O enfermeiro é visto como o profissional chave para o processo de continuidade do cuidado, sendo responsável pelo contato com demais profissionais, organização dos documentos e transportes para transferência do paciente, agenda de exames, além de ser que fica à frente das orientações fornecidas aos pacientes e seus familiares. 7 As orientações dadas aos pacientes e familiares durante a internação e na alta hospitalar devem ser claras, efetivas e considerar as particularidades de cada caso, evitando baixa adesão ao tratamento e reinternações desnecessárias.…”

RESUMO Objetivo: compreender a atuação do enfermeiro para a continuidade do cuidado ao paciente com feridas na transição do hospital para os demais serviços da Rede de Atenção à Saúde. Método: estudo qualitativo, baseado na teoria fundamentada em dados. Foram realizadas entrevistas semiestruturadas com 14 enfermeiros em um hospital ao Sul do Brasil, entre dezembro de 2018 e fevereiro de 2019. Resultados: a categoria central “buscando estabelecer a continuidade do cuidado a pacientes com feridas que enfrentam a transição entre serviços” foi sustentada por três categorias e 12 subcategorias, as quais mostraram que a preservação da continuidade do cuidado e a adequação da transição entre os serviços de saúde ocorrem mediante padronização e planejamento da alta hospitalar. Considerações finais: o enfermeiro destacou-se por iniciativas e pelo cuidado especializado e qualificado às pessoas com feridas dentro do hospital, porém sua atuação no processo de transição e continuidade do cuidado carece de apoio institucional.
