Systemic Absorption and Side Effects of Locally Injected Glucocorticoids

Abstract: Local glucocorticoid injections are often used to treat joint, soft tissue, or spinal pain, but the systemic side effects associated with these injections are poorly understood and not well recognized. There are significant known risks to systemic administration of glucocorticoids. However, there are no guidelines that address issues of systemic absorption, overall systemic risks, or other side effects of locally injected glucocorticoids. For this review, a literature search was performed, and the available ev… Show more

“…Additionally, patient variables, comorbidities, and certain medications can significantly affect the magnitude and duration of HPA axis suppression. Systemic effects of epidurally administered corticosteroids are inadequately studied and underappreciated [ 12 ]. The known protracted dysfunction of the endocrine system from a single epidural steroid injection suggests that parallel systemic effects on the immune response are likely, though this has not been directly studied.…”

“…For example, the median elimination half-life of triamcinolone injected into the epidural space is almost 9 days ( 26 ), causing HPA axis suppression that lasts several weeks. Quantifying systemic dose and risk is challenging, starting with the notion of identifying a threshold of safe number of injections or total milligrams of glucocorticoid injected per year ( 27 ). Prolonged adrenal suppression results in adrenal atrophy, and abrupt withdrawal of glucocorticoids may rapidly lead to adrenal crisis.…”

“…Presently, some glucocorticoids are commonly used in the clinic to prevent this mechanism of neuroinflammation caused by secondary SCI. However, long-term clinical use of these agents diminishes their effectiveness, coupled with significant side effects [ 19 ]. Therefore, modern interventional agents having minimal to no side effects concomitant with favorable therapeutic effects are needed.…”
