As reviewed in the article by Perry and colleagues (2014) in this volume, ample evidence has documented the contributions of peer support (PS) to health, health care, and prevention. Building on that foundation, this article discusses characteristics, contexts, and dissemination of PS, including (a) fundamental aspects of the social support that is often central to it; (b) cultural influences and ways PS can be tailored to specific groups; (c) key features of PS and the importance of ongoing support and backup of peer supporters and other factors related to its success; (d ) directions in which PS can be expanded beyond prevention and chronic disease management, such as in mental health or interventions to prevent rehospitalization; (e) other opportunities through the US Affordable Care Act, such as through patientcentered medical homes and chronic health homes; and ( f ) organizational and policy issues that will govern its dissemination. All these demonstrate the extent to which PS needs to reflect its contexts-intended audience, health problems, organizational and cultural settings-and, thus, the importance of dissemination policies that lead to flexible response to contexts rather than constraint by overly prescriptive guidelines.

Resumo Este estudo verificou a efetividade da promoção de atividade física realizada por agentes comunitários de saúde (ACS) em visitas domiciliares. Trata-se de um ensaio controlado não randomizado com duração de seis meses, com um grupo de ACS que passou por um processo educativo para promover atividade física nas visitas domiciliares para intervir em usuários do Sistema Único de Saúde (SUS) e um grupo controle. Foram avaliados a prática de atividade física e os estágios de mudança de comportamento em 176 adultos (n = 90 do grupo intervenção e n = 86 do grupo controle) atendidos pelos ACS. Foram realizadas análises de associação, razões de prevalência e equações de estimativas generalizadas para verificar diferenças entre os grupos. Não foram observadas evidências de diferenças nos níveis de atividade física e nos estágios de mudança de comportamento entre os usuários dos dois grupos. Os ACS do grupo intervenção realizaram mais visitas domiciliares para promover atividade física aos idosos, pessoas com baixa escolaridade, que não trabalhavam e que tinham doenças crônicas. É importante que os modos de trabalho e prioridades dos ACS sejam repensados para ampliar a promoção da atividade física no SUS.

Peer supporters are recognized by various designations-community health workers, promotores de salud, lay health advisers-and are community members who work for pay or as volunteers in association with health care systems or nonprofit community organizations and often share ethnicity, language, and socioeconomic status with the mentees that they serve. Although emerging evidence demonstrates the efficacy of peer support at the community level, the adoption and implementation of this resource into patient-centered medical homes (PCMHs) is still under development. To accelerate that integration, this article addresses three major elements of peer support interventions: the functions and features of peer support, a framework and programmatic strategies for implementation, and fiscal models that would support the sustained viability of peer support programs within PCMHs.Key functions of peer support include assistance in daily management of healthrelated behaviors, social and emotional support, linkage to clinical care, and longitudinal or ongoing support. An organizational model of innovation implementation provides a useful framework for determining how to implement and evaluate peer support programs in PCMHs. Programmatic strategies that can be useful in developing peer support programs within PCMHs include peer coaching or mentoring, group self-management training, and programs designed around the telephone and information technology. Fiscal models for peer support programs include linkages with hospital or health care systems, service-or community-based nonprofit organizations, and partnerships between health care systems and community groups. Peer support promises to enrich PCMHs by activating patients in their self-care, providing culturally sensitive outreach, and opening the way for partnerships with community-based organizations.
