Systematic Review and Meta-Analysis: Preoperative Vedolizumab Treatment and Postoperative Complications in Patients with Inflammatory Bowel Disease

Abstract: Preoperative vedolizumab treatment in IBD patients does not appear to be associated with an increased risk of postoperative infectious or overall postoperative complications compared to either preoperative anti-TNF therapy or no biologic therapy. Future prospective studies which include perioperative drug level monitoring are needed to confirm these findings.

“…Considering VDZ's gut selectivity, the safety profile has been shown to be favourable . One study using single‐centre data showed that VDZ exposure was not associated with post‐operative complications, including a recent meta‐analysis . However, since VDZ's mechanism of action blocks leucocyte migration into the intestinal lining, wound and anastomotic healing after IBD‐related surgery remains a potential concern .…”

“…They studied 146 patients who received vedolizumab 12 weeks before abdominal surgery and 289 patients who received anti-TNF therapy. However, two systematic reviews and meta-analysis [27,28] did not find increased risks of postoperative complications with the use of vedolizumab when compared to either preoperative anti-TNF therapy or no biological therapy. Studies regarding the use of ustekinumab comparing with anti-TNF therapy also demonstrated no increase in the risk of postoperative complications [29,30].…”
